Achievements
The game has 15 achievements. Two of those achievements (Down the Hole and Hidden Doors) are missable, but can be obtained quite early on in a new save. Most achievements will be covered in this section, except for collection and ending ones.

Down the Hole
Get kicked down the elevator shaft.
Missable achievement only obtainable during the tutorial section. Obtained by defeating Gantley the Usurper during the tutorial and then proceeding towards the elevator to the First City; Soap will tell you the elevator is out of order and kick you down the shaft.

Music Connoisseur
Fund Finnegan’s music education.
You'll first meet Finnegan inthe Sunken Village near the elevator to the Sanctuary. Speak to him, and he will move to the Sanctuary when you start your next run.

Next you need to donate coins to him. All you earned coins disappear when you die, so approach him when you return to the Sanctuary between levels, give him money, and talk to him until he gives you a +1 potion modifier and the achievement pops. (I've kept donating 10 coins multiple times and spent less than 100 coins in total.)

Drinking Habits
Double your potion count through investment.
Give all your potions to Soap in the Sanctuary at the start of your run, complete the Sunken Village, and talk to Soap again to get double the amount back.

Suspicious Deals
Make a deal with Callum.
Callum appears in the northern part of the First City where Rupert (the magic teacher) originally was after you rescue him. Callum offers you a random weapon in exchange for some random penalty (-2 to a random attribute or all your experience). Accept the deal once and the achievement is yours.

Fast Track
Finish Sunken Village within the time limit.
The time limit is 2 minutes and will not start until you step on the first moving platform north of the Sanctuary. Simply rush through the level ignoring enemies. You'll get the achievement and some extra Knowledge when you return to the Sanctuary using the elevator.

Bravery
Rescue Emmet.
Emmet is in the First City before the Many in One boss fight. Talk to him, and he will return to the Sanctuary. (Make sure you don't have Modest City modifier active because he only spawns on the full map.)

Incineration
Have at least three platforms burning at once.
There are traps in the Sunken Village that ignite the platform they are on on touch. The fire then spreads to all adjacent platforms, so find one of these traps, place at least three platforms together, and activate the trap.

Missing Limb
Sever Gwen's hand.
Gwen the Matron is the boss of the Chapel of Revenants (the third game area after the First City). During the fight, stay at the front of her and in the middle so she grabs the platform you're on with one of her hands; attack the hand while she's holding the platform. It should come off when Gwen is down to about 1/3 of hp.
Hidden Passages
Hidden Doors
Uncover three secret passages.
There are three guaranteed hidden passages in the game that you need to find for the achievement. The passages are behind illusory walls you need to attack. You don't have to find all three in one run for the achievement to trigger.

The first one is in the tutorial area right above the place where you spawn. It opens the shortcut to the Sanctuary.

The second secret passage is in the room you find Rupert in. The room is in the First City in the northern part of the map, usually reachable via a narrow passage above one of the traders. The room it leads to contains two treasure chests and Random Spells modifier.

The third illusory wall is in the Chapel of Revenants in the room right before you fight Gwen (the one where the corpse and the note are). Hug the right wall and move up, the passage is right in the corner. It leads to the Broken Dagger modifier that makes the fight with Gwen easier.

The fourth illusory wall is in The Divide. (Thanks Menace-123 for finding it!) From the starting area of The Divide, go counterclockwise (South until you reach the bottom, West until you reach the edge of the area, North until you see a gap on the right where you should see the illusory part on the water). This room contains 2 chests.
Ending Achievements
There are two possible and mutually exclusive ways to end a run in the game.

Cleanser
Shatter the Relic.
The first ending achievement you get. Progress through the game normally and defeat The First One, the boss of the Opulent Sanctum (the fourth game area). After defeating the boss, go north to find a locked gate and Iphis waiting for you. Choose to go back to the Sanctuary with Iphis and attack the relic there to complete the run and get the achievement.

Retirement?
Escape Loot River.
Escaping is the alternate ending of the game. In order to escape, you need to open the large gate in the Opulent Sanctum with five golden keys dropped by five bosses instead of going with Iphis to destroy the Relic.

There are some preparations you need to make to fight five bosses in a run:
  • Obtain the Fake King modifier by defeating Gantley in the tutorial or defeating Gwen.
  • Defeat the First One once to obtain the Central Tower modifier.
  • Enable the Central Tower modifier to get the access to the Glutton's Pit, an alternate area to the Chapel of Revenants. After completing the First City with the modifier selected, go to the Sanctuary and take the door on the right to the Pit.
  • Defeat Houndmaster Vlacheslav in the Pit to get the Deep Pit modifier. (The hounds during the fight can be pretty annoying, so I recommend getting the Cynophobia modifier dropped by shield-bearing knights in the Opulent Sanctum to make the fight easier.)
  • Once you are ready for your escape run, select Fake King and Deep Pit modifiers. Make sure you don't have Modest City modifier active so you don't skip the boss in the First City.
The five bosses you need to defeat during the run are:
  • Gantley the Usurper in the Sunken Village. Usually spawns right before the elevator back to the Sanctuary.
  • Many in One in the First City. Usually spawns in the southern half of the map. (You can also go through the Divide and fight the boss there instead but I consider the First City an easier and much more manageable route.)
  • Gwen the Matron in the Chapel of Revenants. After defeating her, interact with the pit she was in to get to the Glutton's Pit (only possible with the Deep Pit modifier active).
  • Houndmaster Vlacheslav in the Glutton's Pit.
  • The First One in the Opulent Sanctum as usual. You should have all five keys required to open the gate at this point.
After beating the First One, go north as usual but ignore Iphis and interact with the gate instead. You'll be transported to the final boss arena. The final boss is Magister Soap; say no to his question and he will challenge you to a fight. He uses a lot of fire and lighting spells and will attempt to escape using the moving platforms and attack from a distance but overall I consider him much easier than the First One. You will get the achievement and see the credits roll after the fight.
Collection Achievements
The four last achievements require you to unlock all weapons, armor, headgear, and spells respectively.

You will need a total of 3,365 Knowledge points to unlock all the items. Farming Knowledge can be tedious unless you use Flayer of Memories - the sword that always gives you Knowledge points if you kill an enemy using its fully charged attack. With Flayer of Memories you can easily get hundreds of knowledge points per level. To unlock it, you will only need to spend 240 points with Freya: unlock Panopticon (45), Bloodstained Tool (15), Noble's Rapier (50), Amateur's Shortsword (30), and finally Flayer of Memories (100).

Weapons
Weapon Obsession
Unlock all weapons.

You will need a total of 1,345 Knowledge to unlock all 21 unlockable weapons. The option to purchase weapons through Freya becomes available after you complete the tutorial.

Scholar
Unlock all spells.

In order to unlock spells, you need to find Rupert and bring him back to the Sanctuary. Rupert is in the First City in the northern path of the map, usually in a room reacheable via a narrow passage above one of the merchants. Find him and talk to him so he returns to Sanctuary. (Don't forget to hit an illusory wall in the middle of the room he's in.)

Armor
High Fashion
Unlock all armour.

In order to unlock the ability to purchase armor, you need to find Karla in the Divide. The Divide is an area accessible via the Sunken Village only if you have the Fake King modifier active. After defeating Gantley, enter the dark archway on the wall behind him instead of going to the elevator. The archway leads to the Divide; once you enter the Divide, Karla will be standing immediately left of the entrance. Speak with her so she will return to the Sanctuary.

You will need a total of 460 Knowledge to unlock all 7 unlockable armor.

@Menace123: thanks for finding it, I added it to the guide. Your screenshots must be set to private or friends only and I can't see them, so I've added your text description only.

Great guide, would be great if you could add the fourth illusory wall of the divide. Cause I believe the first one might be missable or hard to find after the tutorial. I've shared the illusory wall in my profile, you can use it in your guide. From the starting area of the divide, go counter clock wise (South until you reach the bottom, West until you reach the edge of the area, North until you see a gap on the right where you should see the illusory on the water.) This room contains 2 chests for sure.

Easy way to buy everything with knowledge:
-Use random weapon start rune
-Reload run until you start with Flayer Of Memories
-Use platforms and Charge heavy every enemy in Suken Village besides one (should have a few hundred+ : Bonus if you use the rune to spawn more enemies)
-Die
-Use the knowledge you spawn with in the new run (all of it, leave none even if you cant unlock next, you should have like 2-4 hundred)
-Reload run after spending it all, Youll spawn with the same amount you just spent.
-Repeat+Profit.
