Visit any level when it is already 100% and the pentagram is showing. This will trigger the achievement and a Hell version of the level.

Go to Level 6 and type "666". This will trigger the achievement and a flashlight version of the level.

Go to Level 7 and type "777". This will trigger the achievement and an all spooks version of the level.
