Introduction
I experienced this problem with the leap of faith, and also landing into hay bales and I found out why it's happening.

I discovered why, when I tried to record the problem with FRAPs. I was able to see the framerate was 130-250+ when FRAPs was running.

When I tried to record the problem of dying to leap of faith happening, my framerate would be capped at 60fps.

I could not record the problem. It never occured when I was recording.

Aha.

However, it would happen every time without fail when I was not recording, but only when the framerate was above 130fps.

What's causing the problem?
So the problem?

It's the framerate, there's a problem with collision in Assassins Creed when the framerate is above 130fps.
What's the solution?
The solution?
VSync (or frame rate limiting).

Enable VSync and cap your framerate at 60fps / 120fps either in the Assassins Creed game or in your graphics card's configuration for Assassins Creed.

Your problem will be fixed, I doubt this game will ever have it fixed properly with a patch it, it's likely an engine issue.

When I use Vsync it caps at 50, because for some reason that's what my monitor is natively.

so that’s why I was having problems. Vsync with 120 fps cap fixed it completely. Just sucks that I can’t take advantage of my 165hz monitor in this game.

When I was trying to play the game 100 percent (only ended up playing a total of 3 hours) that was happening over and over and restarting me at a past check point so thanks for clearing that up
