Missable Secrets even with 100% completion

As the title says, the game keeps track of most things but there are still secrets you can miss out on even with 100% completion. That's what this guide is for. This was originally a forum post, but I feel guides remain visible while posts eventually get buried and this information is worth retaining.

  Focus Armor Set[www.powerpyx.com] in the story mission “Inside the Mountain“
  Runeweaver Armor Set can found at Thamur's Corpse - but only upon your first visit

• Hel-bramble blocked chests you can get to before having the blades required to open them:
  1 under the Witch's House, but this one also requires the "Winds of Hel" upgrade
  1 under Tyr's Temple, near the story locked "secret entrance" door
  1 on the Isle of the Dead
  1 in the Foothills just behind Sindri (next to one that requires Lightning Arrows)
  2 up the Mountain, but before the Black Breath (there is another Lightning Arrow chest across the chasm)
  1 just under the Mountain's Summit
  1 to the right of the Oarsmen and before Fafnir's dragon
  1 on the Isle of Buri's Storeroom
  (Some of these have to be looted to hit 100%, but some are not counted so I'll just list em all for completeness... am I missing any?)

  (Especially since one comes from a troll which awakens and is only fightable *after* you've probably already cleared the area and forgotten about it)

• NG+ contains a lot of new items (you might have already known this one)
  Cod of War Set, Zeus Set, Ares Set, new Atreus Armors, PLUS versions of talismans and enchantments, etc

PSA: Retribution Axe Handle Bug

Gonna shoehorn this in here...
The axe handle you get from defeating the Valkyrie Sigrun changes the axe's flight path such that it no longer falls during flight - this makes it a lot easier to hit certain Ravens... but it also can bug out your axe throws at fire pots. Basically, if you are trying to detonate a fire pot and have this equipped it can keep going RIGHT through the pot visually over and over, with the targeting icon indicating it OUGHT to be going off... and nothing ever triggers - just unequip this handle and you'll be able to hit them again
