The achievement iteself
Without a hitch: Complete a run without taking any damage.
It might sound simple and already enough to scare off some players but in reality, it's not impossible.
But with who?
In order to achieve this, you first have to play the game enough to understand enemy attack patterns and boss attack patterns. A great character for this achievement would be Mumba as it has a great starting weapon with high damage and fire rate with great mobility, mobility also helps with dodging most of the enemy attacks.
The tips
Mumba is a great character but you'll also need some items to help you out to clear rooms faster.
Here are some items that will definitely help:

Ancient Tome
Beef ramen-
Block of Ice
Bonker
Choco bar
Comedy
Diet pill
Fear itself
Friday Night Pumpkin
Garlic necklace (For melee enemies)
Gemini
Gold ring-
Jester's cap
Marshmallow bunny
Mitosis
Modding kit 1
Momma's cookies
Polisher
Rainbow Bunny
Signed bat
Scope
Snowglobe
Unreliable Angel-
Weird Magazine

Speed and damage is the key to clear rooms without getting hit.

Someone in the comment has confirmed that blocking damage does indeed help towards the achievement many thanks.
The trick
In the recent midrun save update, you can now close game mid run and continue the run at a later time. With this function you can actually just refresh floors and if you take damage on a certain floor or get hit by chunks at the end, you can simply leave the game and continue at the start of that floor and the hit you had taken before quitting won't be counted.
I wish you gamers out there all the best and I hope this helps!

The easiest way to do it to ensure everything goes well is to force close the game in task manager.

update to my recent comment on 2/17/23 3:24, the "trick" of closing the game to wipe the damage taken does in fact still WORK! the strict condition is to actually "CLOSE" CLOSE the game, like clicking the X on the top right or alt F4 the game instead of exiting the run. If u exit the run, it will still keep the progress of "getting hit" so you in FACT need to literally CLOSE the game. Thanks for the trick man, saved me a lot of time.

idk if they patched it or updated it but, exiting out of ur current run and continuing doesn't wipe the damage that you've taken away. I completed a run where i had to exit and click continue again to refresh the level and didn't get the achievement. IDK if i needed to actually CLOSE the game for it to wipe the fact I got hit or they actually patched it. If someone else can confirm this that be great. good luck anyone that sees this post

I didn't use this, I used meth, monster mumba, and dead eye.
