Bottom line - except for 2 of the weapon colors there's not difficulty requirement.
Tried the link up top. It's out of date. There's currently 130 attachments in game, not including the Internal Battery on the N79 Eva Laser and the Custom Holo Sight on the U1A2 GL Conversion. Each attachment type has 10 attachments.
In regards to weapons there's 55 weapons, including the Heirloom Standoff from the Endeavor Veteran Pack.
There's currently 187 perks in game, including the ability replacers awarded from completing Pathogen, and the perks found in hidden caches (2 for each kit except for Lancer, which there are currently none).
Regarding cosmetics, I'm not 100% certain. So far I count 126 weapon colors, 96 decals, 77 emotes, 64 head accessories and 19 kit skins for each kit (19 skins * 7 kits = 133 total kit skins).

And good analysis of the +Damage/-Fire Rate optics, I hadn't considered that.
As to Manta's thoughts, Effective range would be the distance before Falloff starts, while Range would be maximum range.
The Weapons that are involved with Mission Reward are purely from your Completion Reward, they don't appear in the Caches. I've noted multiple times that on insane I've had Attachments from Park's stock come up in my rewards.
The Insane Difficulty weapons drop purely randomly in your completion. I've had the Kramer AR drop for me, another in my crew got the Gruppa for a first drop.
