Never really wanted to play on insanity, yet you wanna achievement? No problem.

Where is this game saved?

London fight (save 59), not far from rockets.

What else should I know about the save?

It's not perfect. Basically it was speedrun (around 10 hours), so in the end you will have only 2 choices. Companions are Garrus and Ashley, both with N7 Typhoon lvl 10, so they can destroy all heavy enemies without problems. Save includes autosave and chaptersave, maybe those will allow you to "restart" citadel. Havent tested it myself.
