First off...
Before starting, you will need to activate hardmode on a expert world so you can access materials needed to make these items and be able to use and unlock the sixth accessory slot since it's unable to use in a normal world. It is also advised that you have played the game for a long time to be experienced enough to deal with bosses and mobs before rushing ahead.
Armor
These are the following items you will need:
First to start with is the most obvious; armor since it contributes to most of your defense.

Chlorophyte Mask (25 def) - This mask is the highest defense you can get at the moment for your helmet slot. You will need to go to a underground jungle in your world and mine Chlorophyte Ore. You will need about 72 pieces of ore in total since every 6 ore gives a Chlorophyte Bar and 12 are needed to make the mask. These are smelted at a Adamantite or Titanium Forge. Then at an Mythril or Orichalcum Anvil to craft the Chlorophyte Mask.

Solar Flare Breastplate (34 def) - This is the highest defense you can achieve for the body armor slot. The materials needed are 16 Luminite Bars which is about 64 pieces of Luminite and 20 Solar Fragments. This needs to be crafted at an Ancient Manipulator. To aquire this you have to have a world on hardmode. Skeletron also has to be defeated before. You must defeat the Lunatic Cultist at the Dungeon Entrance to recieve the drop of the Ancient Manipulator and then Fight the Solar Pillar event to recieve the Solar Fragments.

Solar Flare Leggings (20 def) - This is the highest defense you can achieve right now for the pants slot. It requires 15 Solar Fragments and 12 Luminite Bars so it's about 48 Luminite pieces altogether. Same as the Solar Flare Breastplate it is made at the same crafting station and dropped from the same bosses.
Accessories
These are the following items you will need:
Accessories are the most difficult to find but they are definitely worth it because they give a lot of defense points due to modifiers and their stats. All accessories should be reforged if not to Warding since that is the highest defense modifier which gives 4 def. It also stacks with the stats the accessories gives normally as well.

Paladin's shield (6+4 def) - This shield accessory can only be found from Paladin drops. The Paladin is a rare spawn in a hardmode dungeon and has a low chance of dropping the shield.

Ankh Shield (4+4 def) - This requires the longest amount of time to craft because the amount of materials needed and the very low drop rate. Firstly a Cobalt Shield is needed which are found in Gold Chests in the Dungeon. 20 pieces of Obsidian are needed which are made from water and lava mixed. Craft the Obsidian at a Furnace to make a Obsidian Skull. Combine the Cobalt Shield and the Obsidian Skull at the Tinkerer's Workshop (which is bought from the Goblin Tinkerer costing about 10 gold) to make a Obsidian Shield. Now the harder bit. To make the Ankh Charm which is needed to make the Ankh Shield you will need the following; Armor Polish which is dropped from Blue Armored Bones, Armored Skeletons, and Paladins. Vitamins dropped from only Corruptors and floaty grosses. These both accessories combine to make Armor Bracing. The Bezoar are dropped from Toxic Sludges, Hornets and Moss Hornets. The Adhesive Bandage is dropped from Rusty Armored Bones, Werewolves and Angler Fishes. These both combine to make the Medicated Bandage. Fast Clock dropped from Mummies, Pixies, or Wraiths. Trifold maps are dropped from Clowns, Light Mummies, and Giant Bats. These both make The Plan accessory. The Megaphone is dropped from Pixies, Dark/Light Mummies, Gastropods, and Green Jellyfish. Nazar are dropped from Cursed Skulls, Cursed Hammers, Enchanted Swords, and Crimson Axes. These both craft to make the Countercurse Mantra. Lastly the accessory which does not need combining, the Blindfold are dropped from Corrupt Slimes, Slimelings, or Dark Mummies. Now craft the ones you can to make the Ankh Charm. Finally combine the Obsidian Shield with the charm to make the Ankh Shield.

Frozen Turtle Shell (*30+4 def) - This accessory works in a different way but makes it the best defense accessory out of all of them since when your healh is *below 50%, it gives you a bonus 30 defense! They are very rare drops from Ice Tortoises which are found in underground Ice Biomes.

Celestial Shell (4+*3+4 def) - The Celestial Shell is personally to me one of the best accessories in the game. Giving you the full bonus of the Celestial Stone and a Moon Shell. To craft the Moon Shell you will have to have a Moon Charm and a Neptune's Shell made at the Tinkerer's Workshop. To obtain the Moon Charm you must have a Full Moon occurring so Werewolfs will spawn and give you a chance of one dropping from them. The Neptune's Shell can be aquired from The Creature Of The Deep. These will only spawn in a Solar Eclipse event. The event can be started randomly or initated by using a Solar Tablet which can be crafted at a Mythril or Orichalcum Anvil with 8 Solar Tablet Fragments which are dropped from Lihzahrds and Flying Snakes in the underground temple. It has be at *night time so you can turn into a werewolf to recieve the bonus 3 defense from the buff.

Celestial Stone (4+4 def) - This is crafted from a Moon Stone and a Sun Stone at the Tinkerer's Workshop. Same stats as one but works both day and night. It really means it always works no matter what time. To obtain the Moon Stone you will have to have a Solar Eclipse so Vampires can be spawned. They are a rare drop. The Sun Stone is a drop from the Boss Golem.

Moon Stone (*4+4 def) - This accessory gives only it's stats at *night, so you will only be able to reach the maximum defense at night time. It can't be a Sun Stone as you need to be a Werewolf at night to achieve the extra 3 defense from it's buff.
Consumable Items
These are the following items you will need:
Buffs are temporary boosts from Food and Potions you have consumed. They have a time limit so they will not last forever until you consume more.

Ironskin Potion (8 def for 5min) - This requires one of each to make; Bottled Water which is made by collecting Sand from Sand Deserts and crafting Glass at a Furnace. Then made into Empty Bottles. Go to any place where there is water and stand near it to make Bottled Water. Daybloom which are found in forests so they are easily farmed on creating a new world or by planting the seeds and farming them when they have grown. Iron or Lead Ore which can be mined underground or sometimes above ground. With the materials go to a Alchemy Station (just place a empty bottle on a surface which counts as a station) to make the Ironskin Potion.

There are loads of other food types that can give you the "Well Fed" buff but it doesn't matter which one you use since they all give the same bonus stat which is 2 def and more. If you're wondering they do not stack with each other since they give the same buff. Although they do have different duration times and the longer the better. My favourite is the Bowl Of Soup so I'll just use this one for my guide.

Bowl of Soup (2 def for 30min) - These can be made at a Cooking Pot. You will need a Bowl which is crafted from Clay Blocks at a Work Bench, a Mushroom which can be found in forests and a Goldfish which spawn in any water source or from the statue.
Buffs
Dryad (8 def) - You will need the Dryad NPC to be available so it can give you a certain buff when there are enemies nearby. She will cause a leaf barrier and give you the "Dryard's Blessing" which gives 8 defense to you character. To spawn her make sure you have an empty house and have killed a boss in that world. The only bosses that don't count are Slime King and Wall Of Flesh.
Now go play Terraria!
This brings the total defense to 176! Nothing shall hurt you! (Slimes will still do 1 damage to you in a normal world.)

I hope you have found this guide helpful. I had to update the guide again due to update patch 1.3.
Just some things you should know; with this setup it really isn't recommended to use all the time because it's not really that good all around. There are so many other combinations of armours and accessories to equip which are much better suited for uses like exploring, mining, pvp etc. Anyways, be creative and do whatever you like, you don't have to follow this setup. This is here just for people who are curious in testing out stuff in Terraria. In addition since we have not seen all of 1.3 throughly yet comment if you think you can get the mex defense higher with other items or buffs.
Have fun guys!
