Method #1. Hosting a dedicated Internet server
Preparation step
In order to make your server visible in the internet server browser you need to do a proper port forwarding. It could be different for various router firmwares.

Just google "port forwarding YOUR ROUTER MODEL"

Keep in mind that you need these ports to be forwarded:
Protocol: TCP/UDP
Ports numbers: 27960, 27961, 27962, 27963, 27964, 27965.

This is how it looks on my router:

Step 1.
Go to steamapps\common\RealRTCW\coop

Step 2.
Create .txt file and rename it to server.bat (you must change file extension to .bat)
HOW TO CHANGE FILE EXTENSIONS[www.softwarert.com]

Step 3.
Edit server.bat with notepad and include this text:
RTCWCoopDED.x86.exe +set dedicated 2 +map escape1
Save edited server.bat file.

Step 4.
Launch server.bat. This will launch your dedicated server with escape1 map.

Step 5.
Try to find your own server through the internet server browser in RealRTCWCoop.
If you see it then you did everything correctly and other people should be able to join your server.
Method #2. Hosting server through Hamachi
If you and your friends still can't see your server in the internet server browser, you can try hosting it through the Hamachi software as LAN server.

Step 1.
Go to. Hamachi website[www.vpn.net]

Step 2.
Press Download now button and download hamachi setup file.

Step 3.
Install Hamachi on your PC.

Step 4.
Launch Hamachi. Press "Power ON" button.

Step 5.
Register or login through your LogMeIn ID.

Step 6.
Press Create a new network and specify a name for your network (e.g rtcwcoop01)
You must also specify a password for your network.

Step 7.
Now your friend must join your network. He should repeat all the steps that you did before but instead of creating new network he should join yours by entering its name and password in Hamachi.
If everything was done correctly it should look like this:

Step 8.
Create non-dedicated server. Setup map, gamemode for your taste and start it.
It should look like this:

Step 9.
Your friend should be able to see and join your server in LAN server browser.

yesterday my friend and I tried out Hamachi. It worked pretty smoothly; the steps are well detailed in this guide :staymetal:
The main problem we had was joining the server in the game. I tried joining my friend's game and I had an error like Invalid cheksum Escape_Castle.pk3 or something like that. I searched the Internet and tinkered a little with game files, and I'm not sure how my friend was able to join my game :jera:

Does anyone know how to deal with endless Awaiting gamestate? We trying to do it via Hamachi.

In short, I didn't succeed with Hamachi, my friend didn't understand how to log in, even though he gave me data and changed it and gave me more simplified ones - it was useless, BUT I managed to do it through RadminVPN and I was ABLE to create a server and my friend successfully logged in, I'm writing from the moment during the game with a friend! Folks, if you can't do it with Hamachi, as I didn't succeed - link to RadminVPN,

P.S. The principle of operation with RadminVPN is almost the same as with Hamachi with some differences. Good Luck!:cozywolfensteinII:

For anyone still having trouble with this. I was able to get the dedicated server working by editing my server.bat to these settings:
RTCWCoopDED.x64.exe +set dedicated 2 +map escape1 +net_ip (your public IP here) +net_port 27960
Still make sure to forward the ports mentioned above. Run the server.bat and then it should show up on the server browser.

It doesn't work, I have fixed ip, opened ports, tried both in-game hosting with radmin/zerotier, and using bat file. I can't see my game listed, friends can't join through console or server browser.

Aha! I figured out how to get it working; had to manually set a fixed IP for my PC in the router and boom now the server shows up.

Question: How do I configure it though? I want to turn the difficulty up at the least. I click the link above but it leads to an outdated page with no info.

Couldn't get it to work with Hamachi, but it works with Radmin VPN without any hassle. So i suggest you use Radmin VPN

Thanks for writing this down

I'd also like to add the following from my experience:
Me and my friend had problems with hosting the server. I did everything according to this (or other tutorials) but my friend couldn't see or join my game. The problem was with Avast firewall. Once I turned it off, everything went smoothly (apart from my friend losing score and ammo after each level - probably due to the LAN connection?).

So, if anybody has the same issue, try turning firewall off if you have a third-party AV like I do. There probably is a setting to whitelist Hamachi/RealRTCW, but I couldn't be bothered after I finally got it running :D
