Also, and this is important -- you need to get leeches on your right arm. This is because it seems like you will always get the first worm on your left arm. The worm overwrites the leech.
I think you probably could get one leech on your leg and then get a rash on your arms from wasps.
Lastly, it doesn't have to be a snake bite. I used a scorpion and it worked just fine. Scorpions are more reliable so keep that in mind.
So, the best way to do it is -- arms: a worm and a leech (just go about harvesting materials/making a base until you get a leech on your arm then sleep on the ground); then snake bite; then armor the three afflicted limbs (leaving your healthy limb exposed); then go play with the ants/bees; finally, eat spoiled meat (no reason to run around with food poisoning/parasites because it takes 3 seconds to open your pack and get them). This assumes you already have mud and insomnia.
Furthermore, you should make ash bandages for the worm wound(s). You don't want to get a fever prematurely and the ash bandage makes it very unlikely that the wound will get infected.

Also, I had a really hard time getting a rash anywhere until I took off half my armor, then BOOM rash right away.
