Introduction
Missing images/achievements
Some images in this guide may be missing. This is Steam's fault. Their databases are f*cked and dozens of images keep getting corrupted, not only in this guide but also in many others. This has been going on for years but Valve is ignoring the issue.
The worst part is that, once it happens to an image, it's likely to happen again. Re-uploading the image, renaming it, even changing the image slightly doesn't help.

I've replaced the achievement images like five times now and it's always a lot of work because of the way the image UI in guides works.
I also noticed that a big part of the achievement section is actually missing and I have no backup or idea when this happened. Since I haven't played the game in years, I can't give explanations on how to get them anymore so I may add default descriptions at some point, but for now I'll leave it as it is.

SPOILERS AHEAD
A main appeal of these types of games is figuring out what everything does.
This guide is full of spoilers which may ruin the fun.
I tried to put the more secret stuff in the back of the guide so you can check the items without scrolling past it. The items are, apart from revealing what they do, spoiler free.

Emeralds
Emeralds are used to unlock items in the main hub. They are also used for boss rush attempts (3 each).
Randomly found throughout levels.

Health, Mana and Batteries

One heart = 2 hp. Used to refill heart containers (your hp bar)

Shield hearts give you 2 temporary hp that cannot be refilled once lost.

Mana is used as ammunition for wands. Wands come with 3 mana containers and each cast costs 1 mana but also spawns 1 mana pickup at where you cast.

Batteries charge your active item.

Keys and Bombs
Keys are needed to open locked doors and chests.
Bombs can open stone chests and safes as well as blowing up rocks and walls to secret rooms.

Pouches
Contains 2 of any of the items above.
Lamps
Lamps are unlocked by completing enough achievements.
In the achievement area, south of the main room, there are four corridors with closed doors at the end of each. The doors unlock when all achievements in one corridor are complete. The lamps are down the ladder behind the door. After unlocking, the lamps are found north of the main room.

NPCs
Most NPCs need to be rescued before they appear in the town and in levels. You'll find them in a cell and need to find the key. The more NPCs you already rescued, the later they appear in the game, i.e. the last NPCs are found right before the final boss.
All NPCs but the Emerald Giant, Beet and the Old Man are required for the NPC Party achievement.

Hub NPCs
Traders
These are found south of the main room.

Left to right:
Hat Trader, Accessory Trader, Active Trader, Weapon Trader
They sell you their respective items for gems.

Mike opens up the boss rush northwest of the main room.

Beet is available from the start and lets you enter a seed for your run. You will only see his leaves sticking out of the ground until you talk to him.

Shops
After freeing, these NPCs appear in runs and sell you items.
Left to right:
Snek: Sells you 3 items for gold, then becomes available for sale himself (pet)
Boxy: Sells you 3 items for 2 keys each, then opens up and gives you free consumables
Gobetta: Sells you 3 items for little money but you get scourged with each purchase.
Vampire: Sells you 3 items for hearts. You can talk to him to trade 1/2 heart for 3 coins.
Roger : Sells you items for bombs

Misc
These NPCs also appear in runs after freeing them, except for the trashgoblin, which will always appear scourged.
Left to right:
Duck: Answer his question right and you get the content of his chest (50% chance)
Elon: Turns your weapon into another one. Gives you +1 scourge
Nurse: Heals you for money.
Trash Goblin: Take his scourge and he'll reward you with a chest.

Emerald Giant
Offers you to take his emeralds. If you accept, you get a random scourge and a ton of emeralds.
Not required for the NPC party achievement.

Old Man
You'll find him in secret rooms and he either gives you a free item or rickrolls you and gives you nothing.
Not required for the NPC party achievement.
Special Structures
None of the names are official.

Rexcalibur
Increases your max health by 2 (1 heart) but removes all your hp but 1/2 heart.
Also gives you +1 scourge.

Gravestone of Gobbo the Great
Destroying it gives you +1 scourge and a scourged version of the weapon or active item you used in your previous run.

Golden Rock
Breaking it gives you 2-3 broken stone orbitals. It also unlocks the broken stone orbital the first time you break it.

?
Seems to heal you 100%. Charmed hearts appear over your head. Not sure what this does.

Safe
Takes 2-5 bombs to open. Contains coins, keys and one item.

Vending Machine
Throw in enough coins and you get an item. Number of coins needed seems random.
Can be destroyed with explosives for a key and some coins.

Charger
Pay gold to recharge your active item. 1 gold per 1 or 2 bars (seems random)

Proto Chest
Proto chests are linked between runs. Weapons stored in them can be retrieved from any other proto chest.
Scourge
Scourge is a mechanic to make the game harder and to improve your score.
Your amount of scourge points determines how much your score increases and how many elites and scourged items appear. The actual scourges (see table below) are curses that affect your gameplay. Getting a curse will always increase your scourge counter by 1, but it's possible to increase your scourge counter without getting a curse.

Scourge will affect items already in your inventory, e.g. your weapons and active item, so they may all of a sudden become scourged and thus can't be unequipped.

A small overview of what scourge does:
  • 10% score multiplier per scourge point
  • Scourged items cannot be unequipped
  • Scourged weapons deal 50% more damage
  • Scourge caps at 10 points
  • At 10 scourge the Burning Knight will always attack you
  • Scourge spawns more elite monsters (colored outlines)
  • The more scourge you have, the more scourged items spawn

How to get scourged
1) Picking up a scourge
This will give you the respective curse as well as +1 scourge point.

2) Picking up a scourged item
Scourged items have a purple outline and cannot be unequipped.

3) Walking through a scourged doorway
Sometimes there are 3 scourges inside and the door won't open until you pick one, sometimes it's just a room with a pedestal or a chest.

4) Breaking a gravestone
Usually found in Castle Ruins. You get +1 scourge for breaking it and it drops a scourged version of your weapon or active item from your previous run.

5) Opening a scourged chest
This will give you a curse as well as +1 scourge point

6) Getting scourged at the dice sculpture

Getting rid of Scourge
The easiest to get rid of scourge is tossing money into the fountain for -1 scourge each use.

Another way is getting lucky at the dice statue (see picture above).

A Scourge Ring will remove all your current scourge points.

Elite monsters
Elite monsters have a colored outline and come in 6 variations. They mainly appear with scourge active, but you may find single ones here and there even without scourge.
Blue: Tougher to kill (more hp and/or more armor)
Yellow: Drop coins on death
Red: Presumably more damage but not sure
Salmon (light red): Shoot a circle of bullets on death
Gray: Teleports on taking damage
Brown: Drops a bomb on death
Looping aka new game +
Looping can be broken down into the following steps:
  • Find the red key in a secret room in level 2
  • Use this key to open the red lock in level 4
  • Get the Pass from the mini game inside
  • With the Pass, buy the bucket in level 5 for 20 gold
  • Fill the bucket with snow in Ice Ruins
  • Use the bucket on the second phase of the Burning Knight on the last segment of his health
  • Fight and defeat the Dark Mage
This may sound complicated, but it's fairly easy. You get to keep the bucket for the second loop, so you only have to repeat the last steps in order to loop again.

Before you start, I recommend to avoid scourge. Scourged items cannot be unequipped and scourge can double item prices, which could mean that you can't buy or equip the bucket (active item) later on.

Find the Key
In Castle Ruins 2 you want to look for a small grey square outside the level bounds.
To find where to bomb, inspect the walls and floor around it. In this case, there is a grey tile to the right of it. If the entrance was on the wall facing down, there would be a bricked-up door frame.

Place the bomb to open the room and take the Red Key inside. It's a melee weapon and splendid against the Old King.

Get the Pass
Defeat the Old King and go to Desert Palace 2. Somewhere on the map you'll find a door with a red lock. I suggest you clear the level with the key before using it.

Talk to the NPC inside the room and pay 8 coins to start the minigame.
Position the claw above the first conveyor belt tile (it's slightly too far right in the screenshot) and press X when the pass is where it is in the screenshot. Don't waste too many tries, you will need 20 coins in the next area, though you will get some from the boss.

Buy the Bucket
With the pass proceed to kill the Pharaoh and descend to the Jungle. You'll notice that the first level is very small. If it's not, the level generation bugged out and you have to start over.

Go in the shop and buy the bucket for 20 coins.

Fill the Bucket
Continue until you reach the Ice Ruins. Go close to any snow walls (the destructible ones) and use the bucket. You now have a snow bucket.

Use the Bucket!
Once you enter the Secret Library, the snow in the bucket melts.

Fight the Burning Knight until phase 2 where he's just a burning skull.

Damage him until his hp is in the last segment of his hp bar and use the bucket.

Be careful not to kill him if you have high damage. Just spam the bucket as it has unlimited uses.
If done successfully, the game won't end and a trapdoor appears.

Fight the Dark Mage
Take the stairs down into the Techno I and engage the Dark Mage.

He teleports away and sends you through several challenge rooms with the goal of reaching him at the end. One hit takes a segment of his hp bar and makes him teleport to the next room.

If you have poison or fire damage, he will usually just tick down and pretty much die instantly.

Either way, when you have killed him, the next loop commences.
Emerald Caves (Secret Area)
Emerald Caves is a new secret area added in the Spooky Emeralds update. It's an enormous dimly lit cave with dangerous monsters exclusive to this area.
This is a high difficulty area where you can quickly get lots of emeralds. It replaces Caves II and you won't fight the Old King thus missing out on his items.

How to Access
In the very first level, look for a small green diamond on the floor. Place a bomb on top of it to uncover the stairs leading to Emerald Caves.

Treasure Hunter
Find a secret room
Secret areas can be opened with bombs. Look for "solid" areas outside the level walls that appear just like undiscovered rooms but with no connection to them. Then look for different ground or wall tiles to figure out where to bomb. Explosive weapons also work.
There is a guaranteed secret room in level 2 (see looping section).

Dodge Overlord
Finish a floor without getting hit
Very easy, once you got the hang of the game. Sometimes the stairs to the next level spawn right next to the level entrance.

Dodge Master
Kill a boss without getting hit
Also rather easy. Use a melee weapon on the Old King. Dodge his first jump and make sure to not get hit, then go in close and chop away. His skulls get destroyed by your swings.

Weapon Free
Defeat a room without using a weapon once
The easiest way is to use the Voodoo Doll, which instantly kills all monsters in the room. Else just dodge attacks while your pets do the killing.

Sharp Matter
Activate 100 spikes in a single run
Has to be 100 different spike traps. Running over the same trap 100 times doesn't work.

Jump Scare
Find a Mimic
Mimics are monsters disguised as chests. They spawn randomly. Finding a Mimic's Tooth dramatically increases the chance for one to appear.

Van No Gogh
Destroy 100 paintings
Can hardly be missed. You destroy paintings all the time while fighting.

Cat without a hat
Find the cat painting
Ultra rare painting.
Seed: RUHCM20I on the first floor in a room to the right.

NPC Achievements
Rescue Operation
Save an NPC
Find an NPC locked up in a cell, then look around for the room with the Cage Key. Take it back to the NPC to free it.

Took me a while to find it again, it's a single use active item with the description: "Reveals the map". Sprite: https://i.imgur.com/ESXupEI.png

Healing to full and teleporting is what the ankhs do so you probably had one of those on you.

I had the cat's ear on me when I got a killing blow. A quick pop up said "sick dodge" and warped me back to the start of that floor and gave me full health.
I'm pretty sure I've had the ear before and died normally, so maybe it's a chance thing.

the effect for "rabbit bullets" when ever you hit someone with a bullet another bullet pops out of them and jumps to the next enemy. It looked like it could chain at least 2 jumps.
