I think Blattdorf's theory, mentioned further down this thread, is correct. The girl seems a bit too detached when meeting her sister in the hospital room, at first I thought she was just catatonic, but her dialogue after her sister leaves is an important clue. She seemingly doesn't recognize her, and might not even comprehend the concept of familial relations at all. I believe the eldritch tentacles flashing in the background aren't there to indicate that you let the entity out with you, but rather that you *are* now the entity, as evidenced by the tentacles disappearing right after. The achievement title itself also alludes to this, referring to the player as "divine". In this ending the cat in the box is very much dead.

She's another version that was decived, you know how the killer version of us cut out our eye when we didn't wanted to sacrifice our finger? My guess is that after losing first eye she still refused to complete ritual so killer version of us cut out her other eye and lock her in cell.
