T-shirts on other consoles
For completion's sake I will also list the T-shirts exclusive to other platforms below. These are not obtainable in the Windows version of the game. They will just be listed for reference.
  • The Legend of Zelda: The Wind Waker: (Nintendo Switch)
  • The Legend of Zelda: The Wind Waker HD: (Nintendo Switch)
  • Zelda II: The Adventure of Link: Complete the CIA Underground on New Game+ (Nintendo Switch).
  • The Legend of Zelda: Majora's Mask: Complete the CIA Underground (Nintendo Switch). BOSCONIAN replaces this T-shirt on Windows.
  • The Legend of Zelda: Majora's Mask 3D: Complete the CIA Underground (Nintendo Switch). BOSCONIAN replaces this T-shirt on Windows.
  • Dragon's Dogma DARK ARISEN: Complete Killer Marathon BEYOND (Nintendo Switch). ROLLING THUNDER replaces this T-shirt on Windows.
  • 零 〜月蝕の仮面〜: Complete Killer Marathon BEYOND (Nintendo Switch)

In the Nintendo Switch version of the game, an "Unlock All" button was added to the T-shirt shop as an apology for a bug early in the game's life. This button easily unlocks every T-shirt despite how they are meant to be unlocked under normal means. I know how the Majora's Mask and Adventure of Link shirts are unlocked properly but I can't tell how the Wind Waker shirts are supposed to be unlocked without the Unlock All button and I can't find the answer by researching online. As such I'm not sure what to do in the Windows version to see what T-shirt replaces The Wind Waker. I also have to finish this version on New Game+ to see what the reward is on Windows. I'll add both as soon as I find out.

Eventually I will figure out all of these specifics and work them into the main body of the guide.
Achievement guide
Electric Thunder Tiger II
When the first level begins, speak to Death and Drive who will explain a little about the game world. This will unlock Be Gentle, 'K?. It's worth mentioning that you have the option of playing co-op anytime with your starting characters, which unlocks Tag Match. Shortly after learning the controls you will be introduced to Bugs, the main enemies in the game, and after a few waves you will find a portable toilet. Save your game here to unlock Ready to Rock.

Eventually you will make your way to the boss Electro Triple Star. When his health gauge reaches zero, you will be given an arrow prompt. Input the right direction to deal the finishing blow. If you play as Travis Touchdown this will unlock Pinfall but if you play as Badman you will instead unlock Out of the Park Homerun. Let the conversation play out and when the level ends you will unlock Electric Fortress.

Back at the trailer, you can buy a new T-shirt from the PC. You may have even unlocked one or two for free if you found the UE Logos in the previous level. Changing either character's T-shirt in the Bathroom Menu will unlock Philosophy of Fashion.

Absolute Zero
Interact with Travis' motorcycle to begin the visual novel storyline Travis Stricks Back. Click through the first chapter Absolute Zero and at the end you will unlock Life is Destroy.

You can move on to Life is Destroy or replay Electric Thunder Tiger II and input the two cheat codes shown in the new archive on Travis' shelf which will count towards an achievement later.

Life is Destroy
Make your way to the boss Mr. Doppelganger. At one of the crime scenes a pile of boxes can be smashed to find an extra life which unlocks Lucky!. When you reach the boss, remember that if you play now as the character you didn't use in the last level, dealing the killing blow here will unlock the other character's finishing blow achievement. Let the conversation play out and when the level ends you will unlock Suburban Nightmare.

After clearing the level, check your faxes at the trailer to learn that Jeane the cat has become trapped in Electric Thunder Tiger II. Replay that level anytime to retrieve her, which counts towards two achievements later for rescues and Skill Chips.

Magic Tiger of the Mound
View the second visual novel chapter Magic Tiger of the Mound. At the end you will unlock Coffee & Doughnuts.

You can move on to Coffee & Doughnuts or replay Life is Destroy and input the level's two cheat codes.

National Census
View the third visual novel chapter National Census. At the end you will unlock Golden Dragon GP.

You can move on to Golden Dragon GP or replay Coffee & Doughnuts and input the level's two cheat codes.

In this level you will be presented with a few races. When the starting countdown reaches GO, accelerate immediately to rocket start into second gear and unlock Firestarter. Keep making your way to the boss the Smoking King and at the end of the level you will unlock Champion Tiger.

After clearing the level, check your faxes at the trailer to learn that Jeane has become trapped in Coffee & Doughnuts if you saved her from the last game already. If not she will appear there after you've saved her from Life is Destroy.

Modern Observation

You can move on to Killer Marathon or replay Golden Dragon GP and input the level's two cheat codes.

Killer Marathon
This is a short level with no real hurdles. Just reach the end to begin another game within the game called Death Drive, which is not at all unlike Asteroids. Scoring 20,000 points here unlocks Shooting Star but scoring 50,000 also unlocks Comet Hunter. Finally, you will eventually be overwhelmed by the mothership boss and unlock Project AAA.

After clearing the level, check your faxes at the trailer to learn that Jeane has become trapped in Golden Dragon GP if you saved her from the last game already. If not she will appear there after you've saved her from Coffee & Doughnuts.

After clearing the level, check your faxes at the trailer to learn that Jeane has become trapped in Serious Moonlight if you saved her from the last game already. If not she will appear there after you've saved her from Golden Dragon GP. Once Jeane is rescued from Serious Moonlight, you will unlock Cat Detective.

Quark-Gluon Plasma
View the sixth visual novel chapter Quark-Gluon Plasma. At the end you will unlock CIA.

You can move on to the CIA Underground or replay Serious Moonlight and input the level's two cheat codes. If you do so and you've entered all other level codes, this will unlock You've Got the Moves!. You will also have all of K's faxes which can be read for In the Know.

CIA Underground
Make your way to the boss White Sheepman. Deal the finishing blow and when the level ends you will unlock The Lambs of Silence. Next talk to Shinobu to view the credits and unlock Return of the Hero and the Spicy (very hard) difficulty, in which you can unlock Stimulating! for beating all bosses on that setting. Shinobu also becomes a playable character with the finishing blow achievement Clear and Lucid. Returning to a level and playing cooperatively with Travis and Shinobu also unlocks Master and Pupil Team.

On the Eve of War
View the seventh visual novel chapter On the Eve of War. At the end you will unlock Killer Marathon BEYOND.

Killer Marathon BEYOND
Make your way to the boss Silver Face. On the way you will have the opportunity to enter the final two cheat codes which unlocks You've Got the Moves!. When you deal the finishing blow to Silver Face, you may unlock Bitter and Delicious, Stimulating!, ADVENTURE!, Feel the Wind and/or Continue or Die if you meet any of those criteria. When the level ends you will also unlock The Lonely Long Distance Runner and even Skill Chip Lover if you were only missing Bad Girl's Chips. Another cutscene will play, unlocking Finally.... With Bad Girl unlocked, dealing a finishing blow with her unlocks Out of the Park Brawl and playing cooperatively with her and Badman unlocks Parent-child Bond.

Cleaning up
Just by playing, over time you will unlock Rookie Debugger by killing 1,000 Bugs, Veteran Debugger for 5,000 Bugs and Legendary Debugger at 10,000 Bugs. Raising all four characters to level 20 unlocks The Main Man for Travis, Grand Slam for Badman, Hands Down for Shinobu and Peak Condition! for Bad Girl. At the very end of it all, unlocking all 50 achievements described here rewards you with Death Driver.
