intellect2147 has also included images of all the Bobblehead locations, which I left out. If my descriptions aren't sufficient, you can try looking at the images. I've kept the numbering the same, so finding the Bobblehead shouldn't be too difficult.

Additionally, Bobbleheads respawn between chapters (so every bobblehead collected in chapter 1 reappears in chapter 2, etc.) so it's not a huge issue if you miss one, but all their locations are listed for completion's sake.
Prologue
"From whence thou camest, thou shall remaineth"
You'll unlock the following achievement sometime during the game - probably during the prologue. It's impossible to miss.
    Mile long club
      • Take a thousand steps.


Bobblehead 1
From the start, run around the back of the bus to find it on the rear bumper.

Bobblehead 2
Run towards the wooden stairs, but go right of the stairs to find it on a lone rock.
    What! These things again?
      • Find a Bobblehead.


Purposely fail the first tutorial jump and fall to your death five times.
    I keep on fallin'
      • Die by a fall 5 times.


At the second tutorial jump, you can repeatedly jump across five times for an achievement now. This isn't necessary though, as you'll probably get this during the game.
    Cliffhanger
      • Hang from a ledge 5 times


Bobblehead 3
Continue climbing up from the tutorial jumps to until there's a lookout area on your left. Find the bobblehead on a bench.

Bobblehead 4
From the previous bobblehead, continue up the hill but stay left. The next bobblehead is on another lone rock. If you're back on the wooden path, you've gone too far.

The rest of the tutorial is extremely simple. Follow the instructions until a short opening title cutscene.

Bobblehead 5
After the title screen, follow the path down and jump across two gaps. Take the next stairs down and drop down from the ledge to find the bobblehead on your left.

Continue making your way down until you need to burn some ropes on a bridge. Do so, but instead of continuing down, cross the bridge to the other end to find the bobblehead.

After reaching the bottom, continue forward to the fence and you'll join the party.

When you regain control, turn to the left to find the bobblehead on a pile of firewood.

Approach the first lady to get your first polaroid photo. Next, with the Instant polaroid camera in hand, take four more polaroids by using the camera on the four people highlighted in yellow. Lastly, go to the back of the area to the group of children. After the cutscene, focus on the raven and pick up your first raven's heart.
    Meet & Greet
      • Introduction Complete.


Enter your bedroom (first room on the left) and look at the photo on the dresser and take the notebook. Open the wardrobe, and change into all five sets of clothes.
    Master of disguises
      • Wear 5 sets of clothes.


Head to the kitchen, and take the bottle of milk from the refrigerator [8] [9]. Find the room with the boxes next to the living room, and examine the Sacred Scroll. Return to your bedroom to start chapter 1.

Chapter 1
"If you build it, they will come"
Wake up, get changed. Talk to Jack in the kitchen, eat your breakfast and exit the house.

Outside, a raven sits on the fence. Kill it for its heart, which you can use to transform into a raven yourself and fly between locations. There's a raven at every location and they respawn on each visit. Kill five ravens and eat their hearts for the achievement below.
    You're my heart, you're my soul
      • Eat five hearts.


After leaving the house, instead of following Jack right, turn left and go around the outside of McGuffin's house to find the Amusement Park. On the map, it's SSW of the house. There's a doorway facing McGuffin's house in the fence that you can use to enter the area [10] [11] [12]. On the right is a "Sultan Tells" machine that you can use for another achievement and progress to another (1/4). Reading the hint itself is optional, you just need to use the machine for the achievements.
    I wish I was big
      • Get 1 tip from the Sultan.

Return to your house and follow Jack right down the road this time to Freeman's place.
Head to the kitchen [13], go through the laundry to the workshop. Take the screwdriver on the bench on the left and the T12C3 on the shelf on the right. Talk to the kids for some information and examine the magazine near them and in the bathroom behind them.

Go up to the second floor [14], and enter the room on the left. Examine the magazine on the sofa until Gary Freeman comments on it, then approach him for a cutscene.

Upstairs, enter the bedroom on the right. The bobblehead is on the dressing table.

Exit Freeman's house and continue down the road to the next building on the left which is the school. After the cutscene, go to the back-left corner and examine the teacher's notes on the shelf [15] [16].

Facing the doors to exit the school, look right and find it tucked behind the door.

Head across the street to the gated area which is the Water Tower. Enter the building, climb to the top and read the papers on the floor [17] [18].

Return to the street and continue to the intersection [19].

From the intersection, follow the street south to the Town Hall. Enter for a cutscene.
Take the mayor's schedule from the small bulletin board and examine the timetable on the wall [20]. Walk to Jack for another cutscene. After the cutscene, examine the trophy in the cabinet, and give the schedule to the mayor [21].

Continue south down the road, then turn left and enter the Police Station. After the cutscene, enter the last room on the left (ladies' bathroom) and take the woman's purse on the sink [22]. Go up to the second floor. Examine the locked evidence room (near Jack) and enter Lieutenant Kimble’s office for another cutscene [23].

Outside the police station, run around the side to find a dumpster. Climb up, then onto the ventilation shaft, and onto the roof outside Kimble's office. Go around the back of the building to find another vent, and cross it to the other roof (mind the gap). Climb through window to enter the evidence room. Or you can fly up there if you're lazy. Take the stamps with LSD from the shelf.
Chapter 1 continued
Continue northeast to the Diner. Go inside and listen to the entirety of the conversation [24] [25].

Go back past the Police Station to the Store, and after the cutscene return Nancy her purse for a stack of 20 dollars [26].

Go south down the road to Gabriel's House and ring the doorbell.

Continue further south to the Church. Turn the cross on the roof above the door as well as the two either side of the door for progress towards "Why can't they think of something new?" (3/5).

Go to the Harbour [27] and approach the boat for a cutscene with Larry Ferguson [28].

After turning left at the house in the harbour, look left to find it behind a box before you turn right towards Larry's boat.

Run west along the southern road until you see the Junkyard. Head inside [29] and listen to Ceefor [30].

Open the map look south of the Junkyard for an unlabelled location. This is the Hideout. Travel there [31].

Go all the way west to Morgan's House (we'll come back to Jackson's Ledge later) and watch the cutscene [32] [33] [34] [35].

Find the fat dude, James, in the house and look in the gap between the couch and the wall.

Enter the bedroom on the right at the end of the corridor, and it's on top of the closet.

Ignore the Treehouse for now and fly to the Gas Station. After the cutscene, enter the garage at the back and listen to the conversation with Bruce [36]. After the conversation, exit the garage through the kitchen to the lounge room [37]. Read the flyer on the coffee table [38].

Go to the Mason Motel for a cutscene [39]. Enter the room one [40] and examine closely at the painting on the right, then the one on the left. Exit the room.

Fly or fast travel to Dr. Green's House, then go west to the Mayor’s House. Run around the right side and turn on the stove through the window.

Return to Town Hall to the mayor's office and turn all three crosses (5/5). Concentrate on the mayor when he returns.
    Why can't they think of something new?
      • Turn 5 crosses.


Follow Jack to the Amusement Park. Use the "Sultan Tells" machine once more (2/4) and pull the switch to restore power to the park. Find the mayor on the merry-go-round and search his pockets for the mayor's keys and a strange coin. Approach Jack to finish the chapter.

Chapter 2
"The burning faith"
At the start of the chapter, use the screwdriver on the robot in your room to receive batteries. Find Jack in the study and talk to him.

Head to the Town Hall and enter the courtroom to listen to the public hearing. You don't have to listen to all of it, but enough for your notebook to update James Morgan's entry.

Go to the Mayor's House and unlock the front door with the keys you picked up yesterday [41] [42]. Use the screwdriver on the "Magna Journey" Gaming Machine in the lounge room and receive the CX145 Processor.

Go to the Mason Motel. Look through the window of room one and to see Banter peeping. (Note from CRASHARKI: concentrate on the curtain to watch the cutscene and receive the journal update.)

Go to the Diner and listen in on the waitress talking with the customer.

Travel to the Church and receive an update to your notebook saying it's still locked. Go around the site and read the note on the cellar entrance. Gabriel's taken the keys for the cellar.

Gabriel’s House is now open, so go inside [43] [44]. Examine all the drawings on the wall and take the keys to the Church's cellar on the chair.

Return to the church and open the cellar doors. Head down and examine the bronze angel with a sword to begin the communion. Try to enter the church, but the power of the cross blocks you.

Go to the Junkyard, and exchange all three things with Ceefor. The CX145 Processor for the Wrench, the batteries for the wire cutters, and give him the T12C3 so he can finish his machine.
    The Machinist
      • Help Ceefor to build his machine.


Follow Ceefor and see his secret project [45]. After he starts the machine up, pull the lever to kill him if you want.

In the garage, in the far corner hidden behind the vending machine. Grab it before killing him.

Return to the Church, and climb the bell tower using the entrance on the side [46]. Walk out onto the roof and use the wrench on the large cross.

Enter the Church [47]. Examine the both photos of the Fathers on the right for cutscenes.

Now wait for your journal to update and it tells you to desecrate some items. On the lower floor, tear down the two banners, and toss the bibles on the floor (3/7). Repeat for the ones upstairs (5/7). Focus on Gary (second person from the left) and then the cross to make him knock it down (6/7). Finally, focus on the cross behind the preacher (7/7). Communion ends.

Run up to the preacher, focus on him and ignite the bible in his hand. Focus on him again and kill him.
    Thy sword, thy preach
      • Chapter 02 Complete.


Chapter 3
"The balance between"
After the cutscene, go to Jack and listen to what he says. After Jack leaves, return to your room and focus on the spill to clean it.

Go to the Amusement Park and use the "Sultan Tells" machine for another hint (3/4).
Search the pockets of the preacher killed last chapter for a second strange coin.

Go to the Police Station and approach Jack and Kimble for a cutscene.

Go to Mason Motel and go around the back for a cutscene.

Go to Jackson's Ledge [48] and use the cutters on the barbed wire along the edge of the road.

Follow the wooden path up to a small lookout area. The bobblehead is under the bench.

Go to Morgan's House and break one of the fuses in the garage. While James is busy fixing the fuse, steal his bottle of whiskey and pour it into the toilet. Watch the ensuing series of cutscenes.
The doctor should die, Derek should run away to the treehouse and Samantha locked in the backyard.
    The Doc could not make 88 mph
      • Kill the doctor.


Go to the backyard and weaken the wooden boards covering the well. Go up to Samantha and lift the door lock. Play tag with Samantha. Tag her then run away for 30 seconds for an achievement. Now run to the well and jump over it. Samantha will fall in.
    Run Lola, Run!
      • Make Samantha Morgan run for 30 seconds.

    Birth of Samara
      • Make a person fall in the well.


Go to the Treehouse and lower the ladder with your powers and climb up [49] [50] [51]. Go to Derek and when he starts swinging the bat, use the raven for assistance.

Go to the Town Hall. Examine the scales in Judge Jackson's office and enter Gabriel's office for an update.

Go to Judge Jackson’s House in the far northeast. Take a bottle of milk from the fridge in the kitchen [52] and read the documents in the dining room [53] [54]. In the study, look at the photo on the fireplace, the painting on the wall with the Latin phrase and take the notepad on the desk.
In the bedroom examine another photo and some newspaper articles.

Return to the Police Station and show the notepad with the Latin words to McGuffin. Your task is to get it translated now.

Go to the School and show the notepad to the teacher.
Go to the Church's cellar and read papers on the table.
Go to Dante Manor [55] [56] [57] [58].

From the front door, follow the path laid out by wooden planks watching for a path that leads right. Follow this path to the very end and look left to see the bobblehead in a safe.

Return to the fork and go straight this time. Before the next T-intersection is a safe on the left with the bobblehead.

At the T-intersection, turn left to find it on a bookshelf.

At the T-intersection, jump through the wall into the garden. Follow along the wall of the house keeping it on your right. Go through a gap in the fence and follow the path to an open area with two small statues. The bobblehead is on the right statue.

At the T-intersection is a book that translates Latin into English. After picking it up, there'll be a long cutscene with Gabriel. When Gabriel starts his story, press ESC to skip it for an achievement.
    The language shall remain dead
      • Get 3 Latin hints.

    Okay. Enough with the monologues!
      • Skip the long monologue of Gabriel.


Travel back to Judge Jackson’s House [59].

Go to the very northeast corner of the map where you can see a tunnel going into the mountain. On the kerb here is the bobblehead.

Back at Judge Jackson's House, enter the study and pull the books in the correct order: Kill them all // for the Lord // knows // those // who are his. Go down the secret path and pick up the dirt on the barrel.

Look on the left for a puzzle. From left to right: fill the bowl with milk, light the candles, fill the bowl with dirt, and blow the windchime. Pull the lever to unlock the gate, open the large doors and examine the judge for another cutscene.
    The scale is even
      • Chapter 03 Complete.

Chapter 4
"Death over the cuckoo's nest"
Read the note from Jack in the kitchen.

Go to the Amusement Park and use the "Sultan Tells" machine for the next hint (4/4).
Search Judge Jackson's pockets for another strange coin.
    ...and they say magicians don't tip!
      • Get 4 tips from the Sultan.


Run down the street to Freeman's House and take the letter from the letterbox.

The following section is optional and perhaps shows the developer intended method of killing the Freeman brothers. It will however lock you out of an achievement next chapter. You can perform the kill to view the cutscene then reload the beginning of the chapter.

Go upstairs in Freeman's House, and enter the room with the three brothers. Give the stamps with LSD to the older brother. Automatically travel to the Hideout and listen to the conversation between the brothers. Focus on the older brother then the staircase illusion. The two younger siblings will follow him.
    No child's play
      • Kill 5 kids.


Reload the beginning of chapter 4 here if you killed the three kids. I'll explain later why it's necessary to keep the kids alive. Be sure to redo the stuff before the first text box.

Go to the Police Station and listen to Jack and Kimble. Examine the missing persons poster. Show Jack the letter you found in Freeman's letterbox. Exit the station and you'll travel to their house.

Go up to the second floor and confront Gary. Go to the workshop where you got the screwdriver in chapter 1 and to the small room on the right with the strange floorboards. Take the crowbar leaning against the chair and use it on the floorboards. Inspect the stash for another cutscene.

Back at the Police Station, use your power on Gary Freeman and focus on the towels next to him.
    True Grit
      • Create 2 deaths by hanging.


Go to the Gas Station and buy an oil filter with the stack of $20.
Travel to the Harbour and give Larry the oil filter.

This is more or less the last time you'll have freeroam of the entire area. The remainder is the game is fairly linear.

Go to the Mason Motel. Go into room one and look through the hole in the wall.

Run around the side of the motel and up the stairs to the house at the back. Approach the window on the left for a cutscene. As Banter exits the house, follow him down to the motel and approach him for another cutscene. Enter room one again and look through the peephole on the right, then the left.

After Banter leaves, follow him to where he buries the body and approach him for a journal update.

Run back to room two and turn the cross on the wall and burn the Bible on the bedside table. When Banter comes back, use your powers on him.
    Death is a game we all play
      • Chapter 04 Complete.


Return to the Amusement Park and search Banter's pockets for the last strange coin. Use the "Sultan Tells" machine for the next hint (5/4).

Go to the Harbour, climb on board Larry's boat and unhook the rope. You head out to sea [60].

In a compartment in the floor. Look under the door that's slightly ajar on the left if you're facing the can of worms.

Take a worm from the can and use it on the fishing rod. Check the rod to catch a fish. Pick up the fish you just caught and use it on the rod. Check the rod again to catch a bigger fish. Use the cutters to gut the bigger fish and receive fish guts. Use the guts with the rod to throw them overboard, then use the lever behind you to kill the fisherman.
    Killing the sailor man. Poop, poop
      • Kill a person at the sea.

Chapter 5
"The burning love of Christ"
Chapter 5 is a very short chapter which is basically clean-up for the rest of the townspeople.
There's an achievement for burning 8 people, but if you kill too many in the first four chapters, there aren't enough left in the church. If you killed everyone then the church will have:
  1. Gary Freeman's wife
  2. Amanda Green (the teacher)
  3. Nancy Morgan
  4. James Morgan
  5. Robert Williams
  6. Lt. William Kimble
Leaving the three Freeman boys alive increases the total to 9, which is enough for the achievement. Oddly, Cheryl and Bruce are nowhere to be seen.

Simply kill everyone with fire and then Kimble last. Super short chapter.
    Burn mother... Burn
      • Incinerate 8 persons.

    Punished by the devil
      • Chapter 05 Complete.

Chapter 6
"Sooner or later, they all turn"
You wake up in Dante Manor. Walk forward and go left and jump over to the next platform. Walk around the corner, pull down the wall piece and jump across. Go down the stairs on the right to the ground and follow the only path.

My game got super buggy at this point.
At the start of Chapter 6, I would always spawn underneath the starting platform.
If this happens to you, turn around from where you start and run through the gap in the wall (see image). Look left and you'll see the two fountains where we picked up bobblehead 58. Take the path you took before back to the T-intersection and back to the entrance of the area.

After the cutscene with Gabriel, go through the doorway on the left and down into the mansion cellar. Follow the linear path to a familiar room if you played the first game. Touch both corpses and retrieve the Dagger of Destiny from the corpse hanging on the table [61] ["62"]. Exit the area.
    I have it all
      • Collect all main items.

Go to the Cemetery [62] [63]. Go to the doors of the mausoleum and use the dagger on the bowl on the left. Pull the lever on the right to open the elevator. Insert all 4 coins into the mechanical device to activate the elevator.

On a headstone in the pet cemetery. It's in the far back left corner of the cemetery, if you are looking at the mausoleum. Check the third grave from the left.

Chapter 7
"Welcome to Hell"
Go down, get out of the elevator and go forward [64].

On the bridge before the platforming section, check in the hands of the second statue on the right.

Jump onto the swinging platform, then the ledge on the left and climb up.
Approach the next edge, drop down to the sliding platform, then jump on the slanted column at the other end, and walk down to safe ground. Proceed forward [65].

After the platforming section, look behind the pillar on the left before going up the stairs.

Climb up the stairs to the gate, pull the lever on the right and go down.

Run forward and wait for the spinning platforms. Jump from platform to platform until you reach the centre [66].

After jumping past the spinning platforms to the centre platform, the bobblehead is at the base of the icy throne.
    Last one! Where's the cake?
      • Find 66 Bobbleheads.


Approach the figure and stab him with the Dagger of Destiny.
    No sympathy for the devil
      • Chapter 07 Complete.

    There can be only one
      • Kill your father.


Ending 01. Take the dagger and stab the final person.
    ...or the angel
      • Kill an angel.

    Take the throne
      • Take the throne from your father.


Reload the latest autosave.

Ending 02. Take the dagger (or not) and take the path out of the cavern.
    Enter the pearly gates
      • Enter the pearly gates.

Glad to be of help, unfortunately that curtain thing seems glitchy, I was forced to restart the chapter because I missed the bobblehead on the junkyard warehouse, but when I tried to interact with the curtain again I had no luck, the game just didn't detect anything to interact with, very odd.

There's a cutscene and a journal entry for the "Look through the window of room one and to see Banter peeping." in Chapter 2, but you need to concentrate on the curtain to watch the cutscene.

My game is broken. At the "look at rime table part and go for cutt scene part. I kinda skipped that step bc I didn't know itbwas a step until this guide but I killed the guy and already unlocked church basement.....do I need to start over? I can't get the cuttscene and I can't get into the police station
