I made this a while ago, and didn't got the chance to upload it, but here it is now. If you want to do this with your old PSX memory cards, it works too, as far as I know, but with different tools, if you want a video on that tell me!

Requirements:

Part 1

*Modded PS2 (Swap Magic or any other tool to read a burned disc, LauncherElf)

*CD to burn LauncherELF latest version into it (program that we will run on PS2 to transfer saves to an USB stick)

*USB stick to move save files between PS2 - PC

*Internet connection obviously to download the following programs:

-LauncherELF: http://psx-scene.com/forums/f113/unofficial-launchelf-v4-42-a-37242/ (The main program)

-CDGen: http://www.zophar.net/utilities/ps2dev/cdgenps2.html  (Program to make the LauncherELF iso)

-IMGBurn: http://www.imgburn.com/ (The program to burn the cd)

Part 2

PS2 SaveBuilder: http://www.ps2savetools.com/wpfb-file/ps2_save_builder_0-8x-zip/ (RAW save file converter to PS2 Emulator)

(Note: I uploaded this file, due to the availability online it was requested, ps2savetools website is down, don't know if it's temporary or not, if there's any copyright issue with this file being up by me, just say the word and I'll remove it. Cheers) Credits for "gothi" from ps2savetools.com for making the tool.

TO DO: (Do it like I did on the video)

Create a new notepad and paste this:

BOOT2=cdrom0:\BOOT.ELF;1 
VER=1.00 
VMODE=NTSC

*save as SYSTEM.CNF

Inside CDGen Program - ON SYSTEM.CNF FILE:

Tick "Fix LBA" 
Type "12231" on LBA

Make sure System.CNF File is on top of boot.elf and that boot.elf updates to 12232 after setting the previous LBA on System.CNF file.

Inside LaunchELF on PS2:

Circle to explore Memory Card 1 or 2.
L1 to change to "GameTitle and more details"
X to select all the files you want to copy to USB
R1 to access menu 
Circle on the copy option to copy files
Triangle to go back
Go to Mass and click Circle to enter it (USB drive)
R1 to access menu again
Circle To paste files
Done.
