Just unlocked All According to Plan..., I've seen sources say it's been fixed since the Nov 2020 Halo 4 update, but don't quote me on that. Point is, it's working again.

I detract my previous comment. It turns out it's just an internet connection issue. The game registers me picking up the Skulls, but because of a disconnect from XBOX Live (because Steam does not like my internet for whatever reason), the achievements weren't being registered to the Steam Cloud servers.

You might wanna recheck that. There's several bugged ones. Primarily the Skulltaker achievements. I know the locations of (most) of the Skulls in this game, but I've run into a problem with getting the achievements for them. For trial, I've picked up the Mythic Skull about 3 times now, with no achievement unlock notification, let alone an acknowledgement of getting it. All I get is the game saying "Mythic Skull Unlocked" and that's it.

The "Look out for the little guys" achievement is COMPLETELY BROKEN. I've done it coop and solo. In solo I made sure I didn't hit any single grunt and it didn't pop up. In coop I didn't even shoot a single bullet, never, nor thrown grenades, nor meleed, nor ran over enemies with vehicles. I let my friend literally do the whole mission alone whilst I was always behind. The freakin achievement never popped up...
