Introduction
Hello there! In this guide, I'll tell you how to fight The Brain of Cthulhu. That should be the third and last time that I tell you that you're fighting him. Nifty!

Stat requirements:
200 Life
20 Mana
At least 20 Defense (Platinum Armor)

Item requirements:
Platinum/Gold Bow
Flaming/Jester's/Frostburn/Unholy Arrows
Crimson Rod
200 Wooden Platforms
The Rotted Fork

Accessory Requirements:
Rocket Boots
Panic Necklace
The Items: Collecting them and using them
The Bow

The Bow is a very important weapon for this battle. Like I said earlier, the more damage, the better. Using a Platinum Bow will be better than a Gold Bow because of the DPS (DPS means Damage per Second). Any other weapon with a higher DPS, like a Minishark, will be a lot better. The Minishark is very, very expensive though, and this guide was meant for earlygame.

The Arrows

Obviously, for your bow to work, you'll need arrows. Lots of arrows. I recommend about 200 arrows. There are 4 arrows that are particularly good with this boss.

The Flaming Arrow
Flaming Arrows have a medium speed with small knockback. The arrow itself deals 6 damage, and then deals fire damage. You need 5 arrows and a torch to make 5 flaming arrows.

The Frostburn Arrow
These arrows are very similar to the Flaming Arrow. It deals damage overtime like the Flaming Arrow. The arrow deals 7 damage, and have more knockback than the Flaming Arrows. The only downside is that it requires an ice torch, and it's slower.

The Unholy Arrow
These arrows penetrate up to 3 enemies at once. The arrow deals 8 damage, it has great knockback, and it's slightly faster than the arrows mentioned above. The only downside is that it can only be found in Corruption worlds from Eye of Cthulhu, and from the Arms Dealer at night after beating Eater of Worlds. To craft, it requires 5 arrows and a worm tooth. Worm teeth are found from Devourers in, you guessed it, THE CORRUPTION.

The Jester's Arrow
Jester Arrows are the best of the arrows. They deal 9 damage, has the most knockback, the fastest of the arrows, and penetrates an infinite amount of enemies. They are not affected by gravity at all. To craft it, it requires 10 arrows and a fallen star.

The Rotted Fork

(Credit to alasg001)
The Rotted Fork is a spear weapon that deals 14 damage on hit. It also has great knockback. It's a tad bit slower than a Frostburn arrow though. The use time is also pretty slow. To get a Rotted Fork, you also have to break a Demon Heart in the caverns of The Crimson.

Crimson Rod

The Crimson Rod is a magic item. It costs 10 mana to use. When you use it, it creates a cloud at your cursor that rains blood on enemies. Each raindrop deals 12 damage.

Example of the Crimson Rod in action

To get the Crimson Rod, you have to break a Demon Heart in the caverns in The Crimson.

Rocket Boots

The Rocket Boots are an accessory. Holding the jump key in mid-air will allow you to fly for 1.6 seconds. You can buy them for 5 gold from the Goblin Tinkerer.

Panic Necklace

The Panic Necklace is an accessory. When you get hit, your horizontal movement speed increases 50% for 5 seconds. You can get them from a Demon Heart in The Crimson.

NOTE: ONLY break Demon Hearts when you are prepared! Breaking 3 of them will cause the boss to spawn on it's own, which I assume you won't be prepared for.
Building the Arena and Vertebrae Grinding
To build the arena, you need 200 wood platforms (You might also want some torches to light the area up if it's underground). You'll also need a big area to build in. Here is a picture of my arena:

For building reference, I placed 20 wood platforms at the bottom, spacing them as I went along. Then I went up about 3 blocks and placed more. I kept going along until I finished. Then I put down the torches.

We're almost at the point where we can fight The Brain of Cthulhu! We just need to do one thing, we need to summon the boss! We can either go to a Crimson Alter with 15 Vertebrae to craft a Bloody Spine, the item used to summon him. (underlined section suggested by lonum) Or, we could break 3 demon hearts. It isn't a good idea to break the hearts, as you can get cornered into a tiny hole. You can however, place a bed near the arena, then use a magic mirror after breaking the third one to get to the arena quickly. But what if you don't have a magic mirror? How do we easily grind Vertebrae?

This is where the Crimson Rod takes place! It also uses a minor bug with it. Also, this is also a reason why it's good to have the battle arena underground.

First, you have to create an overhang on the ceiling. Use any blocks, and try to make it as small as possible! A grappling hook is very useful to get in the overhang. When you're in the overhang, aim down at the ground and fire the Crimson Rod. The rain drops will go below the blocks hitting any enemies! It should look like this ingame:

Another even better reason is that Vertebrae drop the most from the spiders. So, after you get your Vertebrae, you can craft the Bloody Spine at a Crimson Alter. Now it's time to fight the boss!
Phase 1: Rinse and Repeat
Stand in the center of the arena and use the Bloody Spine. You will get a message saying that the Brain of Cthulhu has been summoned (DON'T USE ANOTHER BLOODY SPINE OR YOU'LL DIE INSTANTLY).

When BoC appears, don't fire. He'll teleport somewhere else. The reason why you should not hit him is that he doesn't have a health bar. When he teleports for the first time, some enemies called Creepers will circle him. There are about 25, I'm not sure how much. Each creeper has 100 health. When you kill a creeper, it'll drop 1-2 Tissue Sample and 2-4 Crimtane Ore. Every 5 seconds, the brain will teleport somewhere else. You can tell where he is teleporting when the creepers are going somewhere else. The creeper's destination is where the brain is. Also, the creepers will launch theirselves at you. Go around the arena in a circle to dodge the creepers.

The drops from the Creepers are much more important than hitting the Brain of Cthulhu! If you die, you'll need the Tissue Samples and the Crimtane Ore to upgrade to a Tendon Bow that deals 20 damage, and you can also use them to get great early-game armor. The Crimson armor set gives huge defense, gives 2% extra damage per piece, and the set bonus is a huge increase in HP regeneration.

If you have a bow (this tip goes well with jester arrows), try aiming at the brain. There will most likely be lots of creeper dudes hanging out in there. The Rotted Fork is also a good way to chip damage at the Creepers, but only when they're coming at your face.

When all the creepers are destroyed, Phase 2 will begin.
Phase 2: Adrenaline Rush
If you're at Phase 2 for the first time, you are going to be very unprepared for it. Here, he speeds up to a very fast pace, and only with Rocket Boots and a Panic Necklace will you take little damage. He gains a healthbar of 1000 health. He also disappears and appears every 3 seconds, and he appears very close to you. If you have a bow, just go in a circle around the arena and shoot at him with a bow.

Also, in this phase, BoC will actually get knocked back. This means the spear weapons work good against him. At this point, the best spear weapon you can have is The Rotted Fork. You can actually stand in one place hitting the boss with RF (rotted fork obviously) without being hit, which is a good alternative if you don't have Rocket Boots and/or a Panic Necklace.

After this phase, you've beat BoC! Congratulations! I suggest you try to get all the Crimson tools and armor pieces, they are very useful in the earlygame.

much easier solution, get spine, go to the surface, use a mace for the first stage to keep the eyes away, use boomerang for second. dead brain.
