Stage One: Home Sweet Home

Achievement	Description	Location
Here's Johnny!	Die behind the purple curtain.
Take over the World!	Die under the table with the mouse cage on top.
Horse head	Die in the bed with the white covers.
The End?	Beat the "Home Sweet Home" stage.

Stage Two: Butcher Lake House

Achievement	Description	Location
Do Not Disturb	Die while hiding inside the Garbage can.
Halloween	Die while hiding behind the bookshelf.
Poltergeist	Die on top of the table with chairs stacked on it.
Lucky Pig	Beat the "Butcher Lake House" stage.

Stage Three: Native Forest

Achievement	Description	Location
Big Foot	Die while hiding inside the tree.
Woodpecker	Die while hiding inside the tree trunk that's on the ground.
Phone Home	Die in the white tent.
Delicious	Beat the "Native Forest" stage.

Stage Four: Sick Tower

Achievement	Description	Location
Entity	Die while sitting in the wheelchair.
Banana	Die while hiding behind the food cart.
Alien	Die hiding behind the 1st hospital bed (the one without the green curtain).
Anatomy	Beat the "Sick Tower" stage.

Stage Five: Streamer Studio

Internet Dialed	Die while sitting in the black & green chair at the computer desk.
Spider Head	Die hiding under the cushions of the couch with the teddy bear.
Booger	Die while hiding inside the toilet.
Sad	Beat the "Streamer Studio" stage.

Other Achievements

First Blood

Be the first to die.

Tips?

I found while trying to 100% the game that sticking with the same spot was useful for me in most situations. Just know this can still fail since it is random where the Butcher strikes.... eek!

If you are having trouble trying to beat one of the stages, I would suggest that you do not hide in spots that unlock other achievements. You also should stick with the same spot through multiple games and just keep trying a few times. I can't make any guarantees, this is just something I noticed worked for me sometimes.

It probably took me about 2ish hours to be able to complete all 21 achievements. I think most people could complete this in 1 - 3 hours easily. *could vary though since it is luck based*
