I know installing a Clean Windows could be painful, but almost 100% of the times will fix things you didn't even know were broken.
Don't know when Steam was updated that included the drivers by default, which detects the PS controller.
And about the "Steam Input Per-Game Setting" > "Forced off", Maybe you had done it before?
From CyberGeralt : My PS4 controller has never worked wirelessly with windows 10 and even when its wired, it needs a third-party app called "input mapper" to trick windows into thinking its a xbox 360 controller. Its annoying af tbh.
But... did you at least try this? Is it an official PS4 controller or some generic brand?
Someday, whenever I can and want to, would do a video tutorial just in case.
