Outline
This guide lists all achievements, and gives exact ways to get the keys. There are a total of 5 keys to collect, an each unlocks a new weapon in the Government Lab.
They are the Prism key, Master key, Phoenix key, Savant key and Prototype key. This guide will walk you through getting each of them. At the moment there is no way to open the back door of the lab. We will see what the future brings on that.

After picking up each key, you can quit to the main menu, the missions don't have to be finished.

To unlock the lab door, loot the key off of the psychiatrists corpse at the end of the game.

Note: You can unlock everything without replaying the whole game. Just go to the relevant sections of the game to set up interactions as needed.
Achievements
Here is a list of all achievements, their description, and a short explanation on how to get them. Some triggers aren't fully understood at the moment, but this should get you most of the way there.

The Dragon's tape
Complete The Dragon's tape.

Completed during the course of the game, after picking up the prison tapes in [Chinatown] and playing through the level they grant once you return to your apartment.

Beat the game.

Don't hang up!
In 1962, the Orlons released one of the all-time greatest doo ♥♥♥ hits, Don't Hang Up. Google it. Anyway, you hung up your phone a lot.

At the start of the game hang up repeatedly on your employer until you get this achievement.

The End
Meet your final end in chinatown.

When offered the choice to die in chinatown, take it. Credits will roll, but you can hit continue in the main menu and return to the level to beat the game for real.

Prism key
Most people go right. You went left.

Discussed in the guide.

Master key
You stopped a suicide. With a murder.

Discussed in the guide.

Phoenix key
Found what the robber left behind.

Discussed in the guide.

Savant key
Didn't wake up Leon.

Discussed in the guide.

Prototype key
Fulfilled your secondary goal in the prison.

Discussed in the guide.

Secret hunter
Found all the hidden keys.

Discussed in the guide.

Beatdown
You ruined his fresh track suit.

Defeat V until Snow comes and saves him. Jump+slash down when he charges in red, dodge the blue charges and slow down time when he fires bullets. Hitting him in the back is optimal.

No love for robots
Poor guy.

In [Studio], on the space floor, do not kill the first big robot and continue on. Once you get the key and start backtracking, stop and listen to his monologue until you get the achievement.

Coffin Head
You killed a suspiciously familiar character on the Quiet Hills set.

During the mission [Studio], after beating the Quiet Hills set floor, a Pyramid Head looking robot can be seen walking around during your return walk. Kill him.

Refused medication
This will go well...

Party Animal
Why did you think it was a good idea to give him your Katana anyway?

Give your Katana to V in his limo. When you get it back you should get the achievement.

Funny prank
Kill yourself in an elevator.

Use a molotov cocktail in an elevator in Bunker 1 to kill yourself, but not in the first one.

What a bad idea
Jump off the balcony of the penthouse.

Discussed in guide for the Master key. Description is also pretty clear on what needs to be done.

Psychotherapy
Defeat the psychiatrist's final form.

Not clear exactly what flags need to be set, but basically make all the choices that upset the psychiatrist. Hang up on him, refuse medication, cuss him out. If during the pre-Bunker session he says something along the lines of "I am sad that you are still alive." then you set it up properly. Continue along the game until you encounter the secret boss.

Forbidden Ultimate Danger Tempest-Eyed MAJORDOMO Imperial Barrage Crab, Evixion
Your trap card was activated.

In [Hotel], select the cosplay dialogue options and go with a "children's card game" route, so the receptionist likes you. Having set up that flag, in Bunker 1 when you see her again, choose these dialog options:
-"can't you just let me pass?"
-"I can't show you my ID but..."
-"It's a challenge, alright"
After that, keep spamming the interruption dialogue option until you win with the "You've just activated my trap card" line and you get the achievement.




Prism Key
The prism key is located at the start of the game. Choose the first mission [Factory] and start from the beginning.

After the cutscenes are over, you will find yourself here.


Jump on to the building on the right, then over to the left. The jump is a little high, but attacking upwards boosts you on to it.


Here you can pick up the Prism key from a corpse.
Master Key
The master key is located in mission 2, [Hotel]. You have to play through most of the level normally, until you reach the top of the building.

Breaking down the final door will activate a cutscene, so don't do that.


Pick up this statue.


Open the door, but do not walk through it. Instead, throw the statue straight across. This will kill your target.


You can loot the key off of his corpse.

Note: Jumping off the building gives you an achievement, and it doesn't matter what you do after picking up the key.
Phoenix Key
This key is in mission 6, [Mansion]. You can skip to the part after 'B' to start right at the mansion itself.

Play through normally until you reach this mission.


In the top left room of this level is a criminal. Don't enter his room. Clear out the rest of the level.


You have to wait on this guy for a while, but once he gets up and leaves, enter the room and you can pick up the key.


You can now return to the menu, or rush quickly to the exit as you are about to run out of time.
Savant Key
This in one of the harder keys to get. It is located in mission 9, [Sltr.house]. You have to be careful on this one, as if you don't know what you are doing, you can have to restart the whole mission. The only times you need to be careful are when you encounter Leon, the dude on the TVs.

The first encounter:

Immediately after breaking down the door, jump up and cut the wire. If the doors started closing, restart the room. If this works he won't snipe you in rooms to come.

The second encounter:

Note the cameras above. If you get seen by them, just restart the room. I have marked about where the danger and safe zones are. Roll through the red and land in the green. It takes a few tries, but isn't too hard. You shouldn't have to fight anyone. Just walk through the room rolling twice.

The third encounter:

Same thing as the second encounter. Restart if they see you. After you make it through, go down to the room below the TVs. You will find the Savant Key.

Prototype Key
This key is unlocked optionally during mission 4, [Prison]. There are two ways to get it, depending on your conversation with your psychiatrist earlier in the level. You can skip to your conversation with him freely. The two ways are a darker, easier way and a better, harder way.

The easy way:

Go to your therapy.
Follow these choices:
  1. I spoke to Electrohead
  2. He told me about a drug that sounds like mine.
  3. Do my powers come from the drug?
  4. I don't want this.
  5. No, I don't want the drug anymore.
  6. No.
  7. No.

There are other dialog paths, but the important part is to avoid the drug. This will unlock the achievement Refused medication. Once you load in to the level a voicemail will play telling you to kill everyone. Do just that. Go through the level leaving no man alive.

Once you beat the level and run in to the homeless veteran, make sure you choose to kill him too.

Then once you get to your house you will see the case with the key.




The hard way:

Take any dialog path that gets you drugged. You must now kill no one, including the homeless veteran at the end of the level, and not be seen by any guards. Make sure not to waste your efforts because of him.

Going stealthy can be neat puzzle, but if you don't want to figure it out yourself, a perfect reasonable decision with some of the parts, here is a walkthrough.

For starters, most of the level should be spent in stealth using ctrl/Y/Triangle.

Room 1


You can hide with your interact button in the open jail cells. For the first room, sneak in to the first one while the guard is walking away, wait for the guard to turn around and get past you, then go to the door. Once the next guard is facing away, continue through and drop down to a safe spot on the stairs.


Repeat the same process for the lower level.

Room 2


This room has a bottle and a bell. Pick up the bottle, drop down the stairs, and use it to ring the bell. (I think, it might just hit the ceiling.) This will distract the guard in the upper sector, allowing you to jump up behind him and sneak past.


If you time this right, you can drop down on to the next flight of stairs. Wait for the guard to walk over you, and exit the level cleanly.

Room 3


This is a pretty easy room. Enter the first door you come across.


You can see where the guard is outside on the little TV. Wait for him to walk to the other side of the door, and then exit the side room. You can then exit the area.

Room 4: Outside

This part is awful. I recommend a controller of some kind, as you can use down to crouch without any risk of rolling, which then alerts everyone to your location.

No real walkthrough on this one, as what you need to do is pretty clear: Go left.

Some tips:

If you are in stealth (ctrl/Y/Triangle), no one can see you when you are out of the lights and out of the brighter green cone. You can enter the dull green view cones without any drawbacks.

If you are in stealth and hiding on an object, such as the bleachers or weightlifting equipment, no one can see you at all, even in the light.


You can crouch with Down to hide behind the tables. Note that you cannot move like when you hide in other objects, but you are still completely hidden. This is part of what makes a controller easier.

After lots of tries, you too can one day see the exit.


Continue playing until you get back to your home building. You will see a case that contains the Prototype key.

Weapons
To equip the weapons go to the level [Govt.lab] and interact with the panels on the wall.

Name
Prism Sword
Makes enemy blood come out as random colors.
Sword Of Masters
Releases a projectile when you attack. The projectile has a cooldown timer.
Savant Knife
A shorter sword that attacks very quickly. Rapidly attacking almost allows you to float in mid air.
Claymore Prototype
A much bigger sword that swings slower, but hits wider and through armor.
Phoenix Edge
Similar to the standard katana, but launches flames after each strike.

This guide may expand to help with all achievements, but its 3AM at the moment and I am done.

I have added a section for all the achievements, but it is incomplete at the moment.

Now just a 100% guide. I would still appreciate comments clarifying the exact triggers on some achievements, but it looks like we have a pretty thorough understanding at the moment.

Hey man i was going for the kill everyone prototype key and i forgot that i had to kill the hobo and i let him live and i still got the key so that part of the guide is wrong

i got the prototype key after doing the hard way and then killing the homeless man, although i chose another option, it did the thing so you kill him anyway. But i still got the key

I mean, I *tried* to not kill him, but...
