My gauntlets, hammer and baserker trinket is maxed out.
Basically use the secondary on the hammer to reduce your MP then use secondary on gauntlets to trade HP for MP just enough for you to still be able to use the Berserk trinket once if you fail there should be 3 HP pots spawned at the start of the map so just try again till you get the mark right.
Not a hard achievement to get but as you can't see your HP in numbers or % it can be awkard to judge when your hp level is just enough to use the baserker trinket.
Another methored tho I did not use it would be to get the Baserker trinket and the healing wall in your inventory. Set up the healing wall and then stand away from it, use your Baserker trinket till you can't use it again move to the wall and then just tap the secondary quickly so that your use it the moment you have enough HP to do so.
That strategy would only be sensible to recommend for people who are new to the game, and don't have all of the skills. People who have spent more skulls can easily just lower their own health, by using the Vampire Gauntlets. (This is especially prominent when playing the Weekly Challenge level called "Dark Arts".)
For people who don't have the Vampire Gauntlets yet, they might just be new to the game. Those people might not be very actively trying to get achievements, because they figure that some achievements will be gained passively. Less effort will be spent if they just allow themselves to get those easy achievements before worrying about the others.
I think the problem is just that people don't like berserk trinket.
