Winning Conditions
Several online-guides state, that the goal is to reach 300 000 points in "Excite QTE 2" in order to obtain the "Mini QTE"-capsule toy.
For me, somehow, it seemed to have unlocked after I reached around 259 000 points.
I honestly don't know, whether this is a bug or it might have been lowered in one of the latest patches, but I'm damn sure, that I received this reward.

EDIT: A steam-user noted, that he only got around 219000 points on the PS4, yet still received the capsule toy. So the winning condition, at least on the PS4 might be only 200000 points.
I don't know, whether there are differences between versions, further investigation required and appreciated. Might just be a bug, or there are "hidden" winning conditions, that I just don't know about.
How I did it
So, you might wonder just how the hell one would reach this amount of points?
Yeah, it's a little blunt, but the patterns are NOT random.

Once I noticed this, I just wrote everything down in a text file, left it open on my second screen and just listened to the sound of the QTE-game, while reading off of the document.

It's a bit cheap, but alas, this is no guide to fun.

This list may contain an error somewhere in the middle, that I didn't bother to fix, since I already got the award. The super-expert-segment could be gibberish, as well. But since I didn't need it, it was optional to me to fix it.

Legend to read list (for X-Box-layout):
Read from left to right and then skip to the next row, once at the end.
  • "do" - down (since lacking a creative way to represent "down-arrow")
  • " ^ " - up
  • " < " - left
  • " > " - right
  • "a, b, x, y" - respective X-Box-buttons (that'll be "x, o, square, triangle" on playstation; dreamcast-layout matches the X-Box-layout [just different colours])
  • space between two inputs represents normal input speed, whereas no space means, that they're part of a quick succession of inputs
  • " - " - the dash only appears in a combination of "something-do" and "do-something" to show that
    the "do"-input is part of a quick succession
Good luck, it's easy as ♥♥♥♥ with this!

EDIT:
A greater mind than me would have come to the conclusion, that the letter "V" is a perfectly fine representation of the down-arrow, as pointed out by a Steamuser. Feel free to write your own file for better readability.

Just a a note, I just got the prize with a score of 219202 on the PS4 version, so I'd say the requirement would probably be 200000 pts, though I don't know if there's any difference between versions.

Actually, the sequence isn't random, but to make it look like it is, some mistakes make it change : if someone wants to use this guide that you've written, I'd suggest using the same pattern, and keeping the mistakes, because correcting one would change the rest of the sequence.

Overall, your sequence is correct. There is one mistake in the "expert" part that changes everything afterwards. The super expert part becomes ok once again (even if combos are not the same, it's the correct sequence), but there is one arrow that is setup to make you loose even when you hit the propper button (I tried it countless times, even with other buttons : the super expert is rigged)
