Installing the map
1: Download a map from the internet.
2: Find the map file, it will always be 3 capital letters.
3: Navigate to: Program Files (x86)/Steam/steamapps/common/Star Wars Battlefront II/GameData/ ADDON. If you do not have a folder in GameData titled ADDON, create one.
4: Take the map folder (Remember, 3 capital letters!) and place it in the ADDON folder.
Playing the map
1:Load the game and find the map in you list of maps.
Custom maps should be yellow, like this.
2: Play the map!

Uninstalling the map
Incase you ever want to uninstall the map all you have to do is find the map file from your addon folder and delete it. If you want to easily delete all of your maps, just delete the ADDON folder. Do not delete any other folders or you may risk damaging your game.

Is there a way to add it directly to the game as opposed through the addon folder? Trying to get it working on a dedicated server. It doesn't recognize anything in the addon folder.

Uh, that link the author gave was to just some of the maps on filefront...for SWBF1! Just go to the site proper and you'll have much better luck. *click* [starwarsbattlefront.filefront.com]
Filefront's a bit broken, has been for 3 1/2 years, keep clicking refresh until it displays correctly. Yyou'll know it isn't displaying correctly if a) the most recent file you see anywhere is from 3 1/2 years ago or b) it gives you a *false* message about the file you're looking for not existing. as I say, keep refreshing the page whenever that happens.

One note on mods, if there are more than 500 addon missions the game will crash upon loading (not maps, missions ...such as Nemoidia Swamp CW conquest, for one, and Alderaan: Lakeside GCW conquest, for another...) if you don't want to part with anything you've downloaded, just make a folder called addon2 (or whatever) in the same place as addon and throw your overflow in there. the game won't read from the folder, but the stuff is still right there for your future convenience. The only limit then is disk space.

what extra stuff?

Conversion pack requires some extra stuff, this is just for custom maps that don't require additional mods.
