Things to keep in mind
Before you'll be able to get ending U, keep the following in mind:

-You will need to have the generator running in order to enter the Dungeon.

-You should probably have the hammer in your inventory, which can be found in room 105 after solving a simple puzzle.

-You cannot let Starry tag along with you at all when you initially see her in the basement or you won't reach this ending.

Rejection: Not always a bad thing
First, go meet up with Starry in the basement. Once you are given the choice, ignore your instincts to help the poor soul and reject her invitation instead. It's ok, Rockette lets her down pretty easy.

Once you've rejected her offer, you are free to enter the Well in the adjacent room in order to see Pale Smile. If you've never met Pale Smile before, why not try getting up close and personal with the Janitor?

Now for more rejection! Refuse the offer to play a game with Pale Smile (you probably won't feel as badly about this as you did with Starry). You'll end up in a holding cell. If you've been through this before, simply exit the room via the hole in the wall you made previously. If not, move the stool aside and use the hammer in your inventory to knock a hole in the wall to escape.

Pick your poison. You can either enter the hole at the top (full of enemies) or enter the hole at the bottom (only one enemy here). My preference, oddly enough, is the top. It's much easier for me to avoid the enemies in this room, especially if you stay calm and do not sprint.

Either way, exit the room you chose, and you will find yourself in a darkened hallway.

There is an unlocked room will come into play later, but there's no need to worry about it for now. Continue on the path and unlock the door at the end of it.

In this wide open area, exit through the door at the top of the screen.

Walk over to the set of stairs, which takes you here.

There's a door at the top locked from the other side, so take the door on the left. You are now on the first floor of the Lighthouse.
I like your dress better~
Continue taking the stairs on each floor, until you reach the third floor of the Lighthouse.

Look for the door marked "AG" and enter to meet up with Auntie G.

She'll give you a trip to the dungeon, free of charge! How "sweet".

Exit your new holding cell, and make your way through the crowd of....well, I'm not really sure what they are.

In this area, there are six locked doors.

Walk past the upper middle door and hear the sound of sobbing. It's Starry! Comfort her, or ask about her.

Take note of the lower rightmost door, the one with more sounds of crying as you walk past.

Exit this area, and the next screen, and you'll find yourself in a place known as the "Labyrinth". Avoid the scuba diver and keep to the lower portion of the screen while heading toward the right.

You should see this door.

Enter it and use the computer terminal to unlock these two doors:

Now make your way back to the area with the six doors and go into Starry's now-unlocked cell.

Exit together and make your way back to the Labyrinth....you'll notice Starry mysteriously disappears as you walk through the curtain. Oh well. She does that.

Once back in the Labyrinth, hug the lower portion of the screen and make your way right again, going further this time. Just keep following the lower edge of the screen as you go right, and eventually you will make your way to this door (just past the spot with the toilet and bathtub):

Enter, and notice Starry's dress on the floor.

Try it on!

Now, exit the Labyrinth (going the way you came might be easiest) and back once again to the hall of six doors. Enter the other door you unlocked this time.

The room is dark and rather creepy. Let's just ignore the strange mother/child hybrid and notice the vent on the wall. Crawl through the vent and exit this hallway.

Mirror, Mirror
Head down toward that wide open area again.

Remember the door we unlocked from the other side previously, before going to the Dungeon in the first place? We're going there. Once in that hallway, enter this door:

Be cautious in the room you've just entered, as there is an enemy lurking around. Carefully make your way toward the middle of the room; there should be a small entrance hidden there. Crawl through....

....eventually you'll find yourself feeling like a whole new person! Quite literally. From here, make your way through the horde now occupying the SW apartments (keeping in mind that being cornered by them won't result in a "Game Over" but will force you to re-start this specific section) and toward the entrance to the first floor of the Lighthouse. It's all pretty linear from this point, there's really few options as far as where to go or what to do. Just keep moving forward. For the sake of not spoiling the ending itself, I shall end the guide here.

I'd love to hear everyone's theories on this ending. Starry is such a mysterious figure, isn't she?

you can only get this ending after you've had the event happen that makes the hole appear in the wall of that large brown room, it just happened to me and i was now able to get the ending

i can't for the life of me find that small hidden passage you mentioned in the center of that room, there's also no enemy there. any chance you could post a picture of the passage's location, or am i perhaps doing something wrong? there are some mushrooms in the center of the room and i was wondering if they might be blocking the passage somehow.

Interesting analysis Scriblerian, I think you're onto something there. Rockette's jab at her weight, could be highlighting Starry's figure as that resembling the Venus of Willendorf. A mother goddess, whose form is overflowing with womanhood, sensuality and fertility. But that's just a hunch.

This *could* answer who filed the police report in ending Q.
That's my take. Of course, that explanation seems a little underwhelming for how much guilt the character is shown to feel. And then there's the focus on Starry urinating into a bucket. That might be meant to mirror to the tampon-tissues in her non-dungeon room's wastebasket. Yeah. Starry's bodily fluids are now a very intentionally recurring motif. Only thing that jumps to mind is how the Catholic canon features miracles involving the Virgin Mother's tears/blood/breast milk/etc. Considering the character's Beatific nature in Red Haze, it could be to paint the character as a perversion of the Holy Mother or other female Saints.

:franbow: I mean, her room radiantly lit, with Christian chorals playing in the backtground. :franbow:

There seems to be a connection.

Anyways, awesome guide! Thanks for sharing.
