Hell to the Chief - 3 Secrets
Secret 1
The first secret in Duke it Out in D.C. is in the press/conference room downstairs. If you go behind the stage you will enter what seems to be a maze of hallways. At the end of the hallway is a section with a box of pipe-bombs. That's the first secret.

Secret 2
On the main floor in the blue room there is a picture. Use it to reveal a devastator and the second secret.

Secret 3
In the green room, across from the blue room, there are sunken panels. If you use the one in the first screenshot you will reveal the third secret.

Memorial Service - 1 Secret
Secret 1
Only one secret in this level and it's pretty late in the level. Right before grabbing the red key in the sewer, water treatment area you will see a fence with an opening leading to a sort of waterfall (refer to screenshot 1). Take the water fall down, kill all the enemies and search the wall for a small opening (refer to screenshot 2). Down that corridor will be the only secret for this level.

Nuked Files - 3 Secrets
Secret 1
The first secret can be accessed by heading to the left side of the F.B.I. building. In the street you will see a box that allows you to jump up onto the side of the building. From there you can jump through one of the windows (where the crosshair is in screenshot 1) to enter what I like to call "The X-Files office".

Secret 2
In between the two escalators there is an odd coloured tile. Use it to expose the second secret and atomic health.

Secret 3
In the lab you will find one of the walls explode and reveal some pig-cops (screenshot 1). Inside the rubble you will find a crack on the wall that can easily be exploded with a pipe bomb (screenshot 2). This will lead to the third secret and a shrink gun.

Smithsonian Terror - 7 Secrets
Smithsonia Terror is probably one of the most complex levels in all of Duke Nukem 3D. Below I outlined the secrets in the order that I completed the level. You can probably do these in a different order. The most confusing part of the level is in the shark tanks. Just remember to find the "switches" -- I won't spoil it too much.

Secret 1
In the first floor female bathrooms there is a crack behind the toilet in the stall on the end. Blow it up to reveal secret one.

Secret 2
On the second floor, if you follow the open cooridors you will eventually come to a small glassed display. Inside the display are some plants and an oriental vase. Destroying the vase and walking through the wall behind it will uncover secret 2.

Secret 3
In the "Living Ocean" exhibit, after you use the yellow key, you will eventually make your way to a control station with a computer and a type of server (screenshot 1). Behind the server is a secret door which leads to secret 3. If you follow the secret you can go underwater and claim a RPG.

Secret 4
In the "Ancient Worlds" exhibit, on the second floor, you will see an obvious atomic health trapped behind a hanging monk. If you stand to the left of the cage (area in screenshot 1) you will hear a door open), run down the hallway on the right side of the cage and enter the secret door (secret 2) to claim the health.

Secret 5
This secret is very close to the previous secret. If you head down the hallway from Secret 4 you will come to a "box room". In the room you will see a box with an arrow pointing up (screenshot 1), jump onto the box and fall back down in the middle. Break the vent and proceed to secret 5.

Secret 6
This secret is also close to the previous two. In the same area you will eventually find a narrow hallway with a group of boxes at the end. Half-way down the hallway are three pipes. To the left of the pipes is a secret door containing the sixth secret.

Secret 7
This secret is back on the second floor in the "Humans in Space" exhibit. As you walk through the exhibit you will reach an area that looks like screenshot 1. In the corner, under the raised floor, is a switch (screenshot 2) which tempoarily opens the door in this section and leads to the final secret in the level, a minor jumping puzzle, atomic health, and another devastator.

Capitol Punishment - 5 Secrets
Secret 1
The first secret is in a small alley between the "Bar" and the "Million Dollar" cinema on the main street. Jump over the fence in screenshot one, head down the alley, and take the elevator up. You will enter a small apartment with a shrinker.

Secret 2/3
This secret actually counts as two secrets... for whatever reason. After opening the yellow door and entering the cloak room you will find a lot of receeded walls. The one with the pistol clip has a secret door, open it up for a medkit and RPG.

Secret 4
This secret is in the vents right near secret 2/3. Break open the vent and head towards the fan, break the fan (screenshot 1) and grab the secret along with the devastator.

Secret 5
This secret first requires the jetpack which can be found in the Senate Chamber in the pit around the pillars. Once you have the jet pack you can go back outside where we first started. In the original alleyway you'll see two fans on the wall. Break either one and jump inside. Use the jetpack to get up to the top of the vents and grab atomic health + devastator.

Metro Mayhem - 4 Secrets
If you want to complete all the secrets, and access the secret level of this episode, do not go through the red door. Instead follow the instructions for Secret 4.

Secret 1
After exiting one of the subway platforms (the one with the multiple pig-cops), you'll head up an escalator into a sorta variety store area. Use the cash register to open up the "Duke Nukem 2" poster and claim the first secret along with some extra pipe bombs.

Secret 2
At the station with the broken down train you will find two sets of vending machines (screenshot 1), use a pipe bomb to blow up the wall above them. This will open up secret 2 and an atomic health.

Secret 3
In the arcade, if you use the "Duke Pinball machine" it will open up the nearby vending machine revealing secret 3 and a hidden office area.

Secret 4
At the same platform that holds Secret 2 and 3, you will want to start running on the tracks after the train goes by. You want to run down the tracks in the direction that the train moves. If you bunny-hop properly, you won't take any damage from the track. Eventually you will reach a fenced off portion of the subway, however the fence has a hole in it (screenshot 1). Throw a pipebomb beside the Cola machine to open up the area further. After fighting a crap-ton of octobrains exit the stationary train towards the secret exit.

Top Secret - 5 Secrets
Secret 1
After going through the blue door you'll enter the second storage facility. On the far wall you'll see some boxes stacked in a sort of staircase formation (screenshot 1). Jump onto the top of the box set and look towards the center of the room. You will see a box without a top (screenshot 2). Jump into the box, claim the atomic health and secret 1.

Secret 2
Secret 2 is a bit harder to get to, unless you have the jetpack. Jump from the box with no top in secret one towards the next set of boxes towards the middle (see screenshot 1). From there you need to jump between two boxes in the next aisle (see screenshot 2). Use the brown box to open the second secret and claim the RPG.

Secret 3
After you have used the blue card on the door you want to head back into the original storage room. Behind the brown box in the first large container (the ones where you use the toggle switch to open -- see screenshot 1), is a secret door containing the third secret. This secret leads back to the second storage room which is why you need to first unlock it with the blue key.

Secret 4
This secret is a little hard to explain, but I'll try my best. If you travel through the aquaduct portion of the aztec area of the map (after you time travel), you will eventually come to a portion in screenshot one. If you go too quickly you will miss it and continue to travel through the aquaduct, however if you go slow enough you can branch yourself off in this other direction. Immediately after this fork you want to hug the right side of the wall (see screenshot 2 for the exact location), you will clip through the wall into the fourth secret.

Secret 5
This secret is much easier. Back on the outside portion of the aztec complexes you will find some a mini-gun box sitting on a ledge (left side of screenshot 1). Use the wall behind it to uncover secret 5.

Brown Water - 3 Secrets
Secret 1
At the start of the level when you enter the larger sewer portion you will find a raised section of the wall that loops around to the back portion of the sewer. Climb on the raised portion to the section in screenshot 1 and walk through the wall to uncover secret 1.

Secret 2
Very close to secret 1, where you grab the blue key, you will notice a crack on the wall. Blow it open to uncover secret 2 and some atomic health.

Secret 3
Once you enter the "Pentagon" open the first set of double doors and enter the office. Use the monitor of the United States to uncover secret 3 and the devastator.

Dread October - 0 Secrets
No secrets on this level!! Carry on!
Nuke Proof - 3 Secrets
Secret 1
In the control office above the huge door at the beginning there is a row of computer monitors. Use one of them to reveal the first secret and a med-kit.

Secret 2
This one is really hard to miss. In the "Central Command" area in the blue hallway you can jump into the ceiling about half way down the hall way. You'll see a hidden ledge containing an atomic health and secret 2.

Secret 3
The third and final secret is actually in the "Central Command" area, behind a bank of WOPR (yay WarGames) computers with a holoduke in front. Use the computers to uncover the final secret and an atomic health.

@DynamikThreadZ - Click on the Steam Icon in your drop down window (if you are running Windows. Hover over the little arrow in the bottom right hand corner on the taskbar near the time clock. Make sure the Steam program is running, once it is, click on it, and then click on Duke Nukem 3D - Megaton Edition near the top of the list. You should then be able to choose from the main campaign, classic mode, and all the add-on content.
