Notes on this guide
The intention is to help anyone who is stuck in the rerelease of Pathologic, subtitled Classic HD, get to experience the story and world of this amazing game. It provides minimal gameplay tips and explanations of mechanics, instead offering a guide of where you must go and who to talk to in order to complete each quest. There are already suitable guides for inventory management, the barter system, and combat, so I’ve left those details out. I’ve also put several notes in parentheses about interesting pathing or general programming quirks I’ve discovered in the process of making this guide. I would recommend waiting to play with anything I’ve mentioned thusly after you’ve completed the task normally, as exploiting mission flagging can often rob you of dialogue and information. I hope this guide acts as a support for you when you are well and truly stuck, and does little to influence how you decide to play the game.

If you are stuck, please check your letters, map, and quest log before using this guide.

Also when I mention a percent change, I mean percentage points.
Day 1
in which the Haruspex turns from a rightful heir into a dangerous criminal.

Your health is low, your reputation is worse, and your exhaustion is halfway high. That’s bad. First thing you can do is look down at your feet, and hidden in the brown grass is the pair of dead bodies from the prologue cinematic. One will have a tourniquet and some money, the other will have a knife at 80% durability and some cash as well. Both will have a heart, kidney, liver, and blood vial below all this. Unique to the Haruspex, you can extract the organs of any dead body, and use them for a variety of purposes. After looting the bodies, speak with the birdlike “executors” before you. One of them will mark Vlad Olgimsky’s house, the Lump, on the map, and after conversing with them, head up to the bonfire and watch the scene play out.

Side Quest: “The Filthy Little Toad and the Waifs”
Head to the far end of the warehouse you find yourself in and talk to the young boy, Notkin. Agree to deal with their traitor, and Notkin will mark their last known location on your map, beginning the quest. But before you go to sort him out, exit the warehouse and navigate to the northern half of the district, and find the lair of Bad Grief. Warehouses that you can enter are marked in front with a burning barrel. However, your reputation being in the sorry state it is means that men will attack you on sight. Either stealth around them or simply make a mad dash. Once you enter a house, they stop following you for the moment. Talk to Grief, and agree to help him out.

Side Quest: “The Filthy Little Toad and the Smugglers”
Both Notkin and Grief are having issues with the same doghead. Head over to the location they both indicate. You’ll find him centered between some stones and the cattle range. Tell him off, or kill him, choice is yours. If you tell him off, you’ll get +5% reputation. If you kill him, you’ll lose 10%, but you can take a shmowder off his corpse. Returning to Notkin after killing him will net you 6 bullets and a revolver at 30% durability, while sparing him will get you +10% reputation. Talking to Grief, a dead doghead earns you 5 shotgun shells (or +5% reputation if you deny them), while mercy gets you another +10% reputation. Either way, quest completed for both of them.

When talking to Grief, if you ask about news in town, he’ll mention an ambush at the station. Two pairs of men, one with a knife and the other without, will be waiting for you back at the train tracks where you began. If you feel up for it, you can take the four of them on. You’ll score a total of 9 shells and a fresh shotgun, not to mention the organ sets. You can also talk to Grief about your reputation, and he’ll mention a cemetery lodge on the east end of town, pointing it out on you map. Head there and talk to the gravekeeper.

Side Quest: “The Survivor”
After speaking with the gravekeeper, head over to the house indicated in the Stone Yard and speak with the man inside. You’ll need your health up to about 40% before he’ll perform the operation, taking 30% health but giving you what you need to bring back to Grace. You can trade organs with him for bandages and tourniquets, in addition to looting through his cabinets for supplies. Go ahead, he’ll insist! Package in tow, trek all the way back to the cemetery. Give it to Grace, and watch the scene. Your reputation will go up 30%, and you can demand the man hands over his knife, currently at 70% durability. Quest complete

(For what it’s worth, you can also simply kill the injured man at any point before completing the quest and lose 30% reputation, then talk to Grace about it and lose 10% more!)

Depending on how speedy you are, or how closely you’re following this guide, it’ll be something around 12:00 in game. Finally, head on over to Vlad’s manor. If you arrive before 11:00, a postman outside will tell you that the man won’t be home till then. Enter and talk to Vlad.

Main Quest: “The Killer of Simon; The Killer of Isidor…”
Vlad will direct you next to to speak with his daughter next door, Capella, along with his son, Vlad Jr. who lives down south of here. Capella is closer, so lets start with her. She’ll hand you your list of adherents, and if you ask her about your reputation, she’ll mention Grace and Notkin. Ask where you can rest your head, and she’ll offer her bed, and indicate your father’s home on the map. Now might be a good time to take some painkillers and get some rest.

After this, visit Vlad Jr. in his hovel. He’ll indicate Grief’s lair as a good place to get bad things. Visit Notkin, and see what he has to say on the matters. He’ll mention someone talked to Grief, looking for you. Grief will tell you where he is, but he won’t be back until 18:00, so no need to worry about him for now. After all that, go to the house Capella marked on the north-east side of town. Outside is a guard. Talk to him to try and get in, and regardless of what you say, he’ll return the favor by trying to kill you. Kill him and steal his guts.

Side Quest: “The Family Home”
On your map, a pair of guards will be marked outside the Termitary. Speaking with either of them causes them to attack. Kill them, and retrieve 2 shells, 2 tourniquets, a bullet, some pocket change, and Isidor’s key. With that, return to the house and enter. Upon entering, you’ll be besieged by four rats. Dispatch them before they whittle away too much of you health, and talk to the kid on the table in the far room. He’ll point out Grace’s shack. Quest complete.

(Interestingly enough, you can immediately go to the Termitary guards and kill them without a word, and with the key in hand, open the door behind the guard in front of Isidor’s house. He’ll despawn when you leave.)

Continuing on to complete the main quest, head back to Grace’s house, and she’ll send you along to Murky. After talking to Murky, you should have enough evidence to prove your innocence. Now, wait until 18:00 and visit the Bachelor, residing in the upper level of Eva’s home. Talk to him, and that’s your main quest line completed, along with a 30% reputation increase. Be careful not to be threatening to Eva, or else your reputation will take a hit! Congratulations, you’ve finished day one!

(If you truly wish just to get it over with, and not learn anything, you can simply talk to Vlad Sr. to start the quest, wait till 18:00, talk to the Bachelor and complete it! Could be useful for speed running, as you have to complete the first day quest to proceed.)
Day 2
in which the Haruspex under any circumstances gains more than he loses - for the first and last time.

It all ends in less than eleven days.

Main Quest: “The Inheritance of the Warden”
To begin the main quest, talk to Vlad Sr. after 7:00. Talk to Katerina and Alexander Saburov in the north-eastern part of town. You’ll receive the Burakh’s key, and be directed to Aspity, who lies to the south. Talk to her, and open the chest that appears on the bed. Inside is 4370 dollars, a Brand, 2 black twyre, 2 bloody twyre, 2 brown twyre, 1 swevery, 1 white whip, and herbal recipes #6 and #8. Don’t worry if you can’t carry everything, nothing in the chest despawns. You can even use it for personal storage, if you so wish. Talk to Aspity again, and if you ask about gathering twyre, she’ll point out the three Worm huts on the map for a side quest later. From here, you can go visit the Bachelor, then visit Vlad Jr. You’ll be directed to a warehouse in the Factory district. Create a twyrine extract, any sort at all, and bring it to the Bachelor. Main quest completed.

Side Quest: “The Herbsman Bride”
To begin this quest, talk one of the Worms at the middle hut, south of the railway station. Agree to look for his daughter. She’s located in the eastern hut, by the cemetery. If you allow her to remain, she’ll give you 3 swevery, and 3 white whip. If you implore her to return, her father will gift you herbal recipe #9. Either decision will complete the quest.

Side Quest: “The Brown Twyre”
To start, talk to Andrey in his pub. He wants 10 brown twyre in exchange for 4000 dollars and the location of a shop that buys items at a premium. If you have the herbs, you can make the trade and complete the quest, or….

Side Quest: “Plain as Day”
If you return to the pub after 30 minutes in-game, 2.5 in real time, Maria will be waiting in the bar. She’ll offer 5000 currency and herbal recipe #5 if you tell Andrey to stay. Choice is yours, although, the shop Andrey is referring to in the clothing store on near the Kain’s manors, so you probably want to extra money and to keep your herbs. Choose an option, and quests completed. Just be warned, if you tell Andrey what you were talking about with Maria, you can say goodbye to that option.
Day 3
in which the Haruspex uncovers the intricacies of the secret Steppe code at a great personal risk.

It all ends in less than ten days.

Main Quest: “The Brand”
To start the main quest, talk to Vlad Sr. after receiving his letter at 7:00, and agree with his request. After talking with Vlad, seek out Aspity once more. She’ll point out a house in the north-eastern part of town for you to visit. Make your way there, and search for the butcher. He is located upstairs, through the door in front of you, then in the room to your left. You’ll then be directed to return to Vlad Sr. Talk to him, and then the main quest is completed.

Side Quest: “Dead Tissue”
The Bachelor will send you a letter at 10:00, and want to speak with you, although you can talk to him before that. He’s looking for infected tissue of any sort. Either you can take the infected blood, liver, heart, or kidney from the corpse of a burlap wrapped person in the infected district, or you can go to the cemetery, and kill the patrol man, and loot the blood off the burning corpse. You suffer no reputation penalty for killing the guard, and you can get a bullet and 12 alpha-tablets from his corpse. Whatever you choose, return to the Bachelor and pick your reward from the following options. Quest complete.
  • Immunogens: 5 beta tablets and 1 delta tablet.
  • Painkillers: 1 novocaine, 1 morphine, 1 etorphine
  • Antibiotics: 1 feromycinum, 1 neomycinum, 1 monomycinum
  • Bandage Packages: 2 bandage sets.

Side Quest: “Capella’s Troubles”
To start this quest, talk to Capella at the Lump. She’ll implore you to check on Notkin in his warehouse. Upon reaching the warehouse, a pair of idling muggers wait outside. You can approach them and chat with them, they won’t attack if you tell them you’ll leave. There’s two routes you can go from here.

You can choose to fight the muggers, either triggering them with their dialogue, or attack one of them. Kill the pair of them, and the door will unlock. Don’t forget to loot the bodies! Once inside, you can talk to Notkin, and he’ll offer you a reward, a shotgun at 30% durability. Accept it, or refuse it to gain +10% reputation instead. Be sure to speak with him again to start another side quest. After that, return to Capella, but be warned, another pair of muggers are waiting for you outside. Sneak past or fight them, and return to her, and get 3000 dollars from Capella, or refuse, and get absolutely nothing.

The other option, for the less combat inclined, after speaking with the mugger, go talk to Bad Grief, who will then ask you to talk to the Bachelor. Once you speak with him and gain his cooperation, you can now return to Grief, and give him the Bachelor’s word. You can check up on Notkin, harassing the muggers a bit too, if you like. Make sure to speak with Notkin to start another side quest. Once you talk to him, talk to Capella, and she’ll offer you only 1000 bucks for your efforts. Either option you choose, quest completed.

Side Quest: “...By the Burakh Family Recipe”
Notkin will ask for your help, requesting ten Twyrine Extracts from you in exchange for giving you maps of the infected districts. If you do complete this quest, you can purchase maps each day for somewhere between 500-2000 dollars. If you don’t, you’ll have to exchange crowbars from looters for it. Personally, I prefer to complete the quest. I find that crowbars are better used for trading, and you get enough money killing bandits at night anyways. The exact costs will be noted below. The easiest way to finish this quest is to make extracts from a single herb. They won’t be very effective, but Notkin doesn’t care if you’re giving him ineffective medicine to protect children in plague zones. Extracts that do the same percentage of both effects stack in your inventory, so don’t worry about dumping the organ horde you’ve been amassing. Do, however, stow away extracts you don’t wanna trade, just to avoid Notkin taking the good ones from you. Return to him with ten extracts, and trade them away to complete the quest.

Day 4
by the end of which the Haruspex makes a friend and finds the half-dead enemy.

The Powers That Be are getting increasingly worried. It all ends in less than nine days.

You’ll receive a letter from the Bachelor in the morning, after 7:00. Talk to him to begin the quest. Once you finish the main quest, however, you’ll be automatically jettisoned to the next day, so don’t complete it before attending to your side quests. We’ll discuss those first.

Side Quest: “The Looters”
Talk to Notkin in his warehouse to start this mission. He’ll request four crowbars for a bounty. Crowbars can be found on the corpses of looters inside houses in the yellow tinged (black on Notkin’s maps) abandoned districts. They’re a pain to deal with in melee, but a single shotgun blast is typically enough to take them down. Point, shoot, and loot until you have 4, and return to Notkin. He’ll give you 2 revolver bullets, 5 shotgun shells, and 10 rifle rounds

Side Quest: “The Weapons Stash”
Talk to Sticky in his house on the north eastern part of town. You’ll meet Stump just outside the gates of the cemetery, due south from Sticky’s house. Don’t, uh, worry too much about the password. Follow the kid, but beware of 3 bandits lying in ambush. Sneak past them if you wish, they’ll freely ignore the kid. Stump will lead you to house second left of Aspity’s hospice. Use a lock pick to open the door. If you don’t have one, you can leave and return to the house later. Pick the lock and find the weapon stash, comprising of a shotgun at 100% durability, and 10 shell. It’s upstairs, on a bed. Quest complete, don’t even have to return it to Sticky. He’ll just tell you to keep it if you do.

(If you like, you can start the quest and immediately break into the house to retrieve the firearm, circumventing the bandits and Stump altogether.)

Main Quest: “The Mysterious Blood”
The Bachelor will send you along to Lara Ravel at the Shelter. She won’t speak with you unless your reputation is at 70% or higher, though. She’ll mark a warehouse in the same district as your hideout. If you go to Rubin’s Prosectorium instead, you’ll find it abandoned, although you can find some “dead tissue” in the desk, which you can bring to the Bachelor for fun, but not much else. In the warehouse Lara indicates, however, you’ll find Rubin. Speak with him to complete the quest, and after depositing everything classified as a weapon in the chest next to the door, exit.
Day 5
in which the Haruspex gets a chance to grab Her Highness the Elusive Sand Plague by the tail.

You cannot fight this. It all ends in less than eight days.

If you completed the main quest yesterday, you’ll wake up in a cell at 14:00 with 30% health, and with your reputation dropped to 20%.

(Interestingly enough, if your health is below 30% or your hunger above 80%, you’ll have your values “healed” to that amount, but if your reputation is below 20%, it’ll stay there.)

A dead body lies outside the cell, and plague clouds are rolling in, so it would be in your best interest to move quickly. On this body is 2 beta-tablets, 1 gamma-tablet, a fully repaired revolver and three bullets, and herbal recipe #10. Fire at one of the other guards, or punch them if they’re close enough. Once agro’d, they’ll open the door to subdued you. The revolver makes quick work of them. Both bodies have 4 bullets, 1 meradorm, 1 morphine, and 3 alpha tablets each. But even if you make quick work, it’s nearly impossible to avoid infection. Head down to the right to exit. Stop by Rubin’s hideout and grab those weapons you so thoughtfully stored away. Until your boost your reputation, guards will attack you, but pedestrians won’t.

(If truly you wish to avoid this sequence altogether, let midnight pass while the you’re in Rubin’s lab, and the guards won’t appear outside. But considering how short the quests are this day, along with the fact you get a free 14+ hours of not having to worry about hunger, I’d say it’s worth it. You were gonna get infected sooner or later.)

You have a letter. Two, actually. One is a bit more important than the other.

Main Quest: “The Quivering Heart”
The Bachelor will implore you to find a fresh heart. There are a few ways this quest can go.

Option 1: Go speak with Aspity. Tell her that this is your decision, and she’ll point out the gatherer tent on the far west side, south of Eva’s home. Now this will split into two more paths.

1.1 Kill the herb bride. Extract her heart and find herbal recipe #4, plus a 50% boost to reputation. Give it to the Bachelor for about 2000-3000 money and a blue vaccine. Talk to him again to complete the quest.

1.2 Kill the butchers. Take one of their hearts to the Bachelor. Receive somewhere in the ballpark of 3000-4000 money and 2 black vaccines. Talk to him again to complete the quest.

Option 2: Go to Andrey’s tavern. Talk to the herb bride named Willow, and she’ll mention that she’ll be meeting the Bachelor behind Peter’s house at 21:00. She actually be there immediately after talking to her, so no need to wait. An hour after talking to Willow, you’ll receive a letter from Anna Angel, who lives nearby. If you kill Willow, you’ll lose 10% reputation, and can take her heart. Talk to Anna for a monetary reward, and you can agree to get 3000, or press her a little for 10000 bucks. The Bachelor will give you no reward for her heart, and the quest will be completed.

(Maybe she’s just jealous she shares the name of her house?)

Side Quest: “The Well”
Talk to Vlad Jr. in his boarded up home. If you disagree over the price, you can get 14000 bucks in advance, along with 50 kerosene, and you’ll be deposited in the well. Get through to the cobblestone endpoint, and talk with the person at the end of the tunnel. After that, look towards the ceiling for a sewer lid, and exit. If you do not talk to this person before exiting, you will unable to complete the quest. After speaking to them and exiting, return to Vlad Jr. He’ll give you 7000 dollars in addition to herbal recipes #2 and #3, and quest is completed.

Day 6
by the end of which the Haruspex finds out just how important a role mythical creatures can play in our lives.

It all ends in less than seven days. The emissary arrives tomorrow.

Main Quest: “Bull’s Blood”
Talk to the Bachelor after 7:00 to begin the main quest. 30 in-game minutes after talking to him, you’ll get a letter from the Changeling, who is waiting for you at Grace’s lodge. Accept her help, and head on over to the Short Block of the Termitary. Head up the five flights of stairs, and talk to the young girl flanked by butchers. She’ll ask for 50 alpha tablets. Time to raid the stores, burn through your money, and trade everything away. Once you have 50, return to her and give them to her, and she’ll give you herbal recipe #1. Head out to the place she marks on your map, beyond the graveyard. Talk to the worms, and hand over any infected organ. Watch the scene, and then take the bull’s blood, and return to the Bachelor. Hand over the blood, and speak with him again to finish the quest.

Side Quest: “The Changeling Seeks an Answer”
You’ll receive a letter from Anna requesting your presence at her home, the Willows. There, you will find the Changeling. Speak with her, and agree to help to start the quest. Behind the left side of the theater is a manhole, which you can enter. As with the sidequest from the previous day, there will be the Rat Prophet before you. Speak with him, and then return to the Changeling at Anna’s home. Whatever you tell her, you’ll receive herbal recipe #7, and the quest is completed.

(For whatever reason, there are static plague clouds in the tunnels. Not sure why, as you’re given no reason to delve back into the depths.)

Side Quest: “They Call the Mistresses out”
You’ll receive a letter from Victor Kain, and after speaking to him, he’ll implore you to talk to Maria Kaina next door. Speak with her to start the quest. Go to the graveyard immediately north, and enter the tomb at the peak. Pick up the diary from the floor, and exit to be greeted by a pair of arsonists attacking a group of infected. Nothing special is to be gained from killing them, so unless you’re feeling inspired, just go the long way around them and return to Maria. She’ll give you 2000 dollars, and the quest will be completed.

(Also, you can re-enter and exit the tomb to simply despawn all of them, and return to Maria unperturbed)
Day 7
fateful - which is to say, crucial and life-changing - for the Haruspex.

Even an Inquisitor will not help you defeat this enemy. It all ends in less than six days.

Main Quest: “Neither Man Nor Bull”
Talk to the Bachelor after 7:00, and he’ll advise you to go to the Cathedral, where the Inquisitor now lies. Expect they’re not here now. An executor will direct you to the Long Block of the Termitary to see Taya. She’s on the third floor, down the hallway lined with six torches. Talk to her to learn that the Inquisitor has departed for the Town Hall jail, and talk to her again to begin the side quest “The Sacrifice”, which you can take care of now. Head to town hall. Facing the pair of doors, it’s the left one. Talk to the guard, and he’ll inform you that the Inquisitor is no longer here, and has gone off to a familiar looking house in the Stone Yard… Upon entering, head upstairs and talk to the young man. The Inquisitor isn’t here either. He’ll mention Victor Kain quite a few times. Head on over to his house, and talk to him. It seems the Inquisitor has now returned to the Cathedral. Return to there, and after this borderline wild goose chase, she’ll finally be there. After a conversation with the Inquisitor, you’ll be informed that the Abattoir has been opened up. Haul yourself back across town, and past the doors of the Termitary, and into a stone arch of the Abattoir. Weave your way into its depths, and speak with Oyun, the Foreman of the Abattoir. He will give you Abattoir Blood. Head to your lab, and mix the blood with any twyrine extract. Return the result to the Inquisitor, and talk to her to complete the quest.

Side Quest: “The Sacrifice”
You’ll receive a letter from Capella, and she’ll suggest checking up on Taya. Speak with her in the Long Block, and she’ll ask you to check on some odonghes hiding out in the Short Block. This time, there’s plague clouds dotting the stairwell, one of which is impossible to get around. Pop immunity pills, throw on your finest clothes, or otherwise just trot through it. Speak with them at the top, then head back down, through the clouds again. A pair of bandits will be waiting in ambush between the Termitary doors, either fight them or book it for the Long Block. Return to Taya, and speak with her to complete the quest.

Side Quest: “Testing the Panacea”
Speak with the Inquisitor after completing the main quest, and you’ll be implored to test the panacea. You’ll be asked to head over to the theater, and speak with the executor inside. Once you’re ready, he’ll infect you to 50%. Head back to the Cathedral, and cure yourself. Speak with the Inquisitor, and the quest will be completed.

(Some notes on this quest; first of all, there is no reward, you use up your panacea and get nothing out of it. Second, if you have a shmowder, you can use that and get the same dialogue. Thirdly, if you cure yourself but get reinfected on the way to the Cathedral, you’ll need to re-cure yourself with another item to finish the quest if you did not confirm yourself clean with the executor. Fourthly and finally, it makes no difference if you’re infected before the quest begins, or get infected between talking to the Inquisitor and the executor, as you must get infected by the executor to complete the quest. Seeing that there is no reward for this quest, and you use the panacea, it might seem worthless, but I think day 7 is a good day to give yourself a fresh start if your meter has gotten too out of hand. But don’t bother to cure yourself of the pest again till you’re at the end of the game. Also, speaking to the Inquisitor and the executor after getting infected but before curing yourself results in some amusing dialogue.)
Day 8
in which the Haruspex gets the idea of who He Who Was Marked by the Secret Sign may be.

It all ends in less than five days. The army arrives tomorrow.

Main Quest: “Whose Blood Was It?”
Speak with the Inquisitor after 7:00 to begin the day’s quest. She’ll ask you to go to the Abattoir, but it’s currently locked up, so you’ll have to speak with Taya in her room in the Long Block to access it. Taya will ask you to find the Changeling, and suggests you ask Capella where she could be. Talk to Capella, and listen to her plea, then talk to her again to ask about the Changeling. She’ll point you to Murky’s van. There you will find the Changeling, and if you tell her that the Souls-and-a-Halves thought the story was funny, she’ll tell you how it went, and if you keep asking for more, she’ll tell you the ending. Make sure to pay attention! If you just demand she tell you the ending, she’ll refuse, and you’ll fail the quest. Return to Taya, and make sure to accurately tell her the story, or else you’ll fail the quest (Option 3, then 2, by the way!). With the Abattoir opened, go and speak with Oyun. He’ll mention something missing from your inheritance, so go back and speak with Aspity. Question her about your inheritance, and she’ll give you the Bull’s Horn. Return to Oyun, and after speaking with him, talk to the Inquisitor. Quest completed

Side Quest: “A Son Will Not Be Punished For His Father’s Sins”
To start with quest, speak with Georgiy Kain at the Crucible. He’ll ask you to speak with Katerina. Once you speak with her, you can go to Taya, and make a decision. Return to Georgiy. If you agreed with him, he’ll give you 3000 money. If not, nothing. Either way, quest complete.

Side Quest: “Below the Ground”
Speak with Vlad Jr. in his hovel to begin this quest. You’ll be implored to return to the tunnels below town and find his book, expect you can no longer enter from his well. Instead, enter from the manhole near the theater. When you enter, be warn, four bandits are placed in the tunnels, and the door is locked behind you. Tread carefully, but be sure to kill them all; they have incredible amounts of loot in the form of around 2000-2500 money and average of 3 pieces of ammo. Maybe even a gun if you’re lucky. Proceed to the place you entered from on day 5 to find Vlad’s Notes, and pick it up to unlock the door. Go back, and speak with Vlad Jr. to get 4000 bucks and complete the quest.
Day 9
in which it becomes apparent that the Haruspex will have to fight for the Kin and the land of his ancestors.

The town is dying. It all ends in less than four days.

Main Quest: “The Defilers”
Speak with the Commander at Town Hall, and he will direct you towards the Abattoir, and Oyun. You can’t start this quest until 7:00, so get some rest and then go talk to Oyun. You’ll be asked to find the Bachelor and question him. Once you begin this quest, your exhaustion will skyrocket, reaching 100% in an in-game hour. Any attempts to combat it are ineffectual, so just keep moving and keep an eye on your health. Going to Eva’s manor will reveal that he went to the Polyhedron, and going to the Polyhedron, you’ll meet a doghead at the base who will inform you he’s been taken to the same prison you woke up in on day 5, near your workshop. If you spared the doghead on day 1, he’ll open it for free. If you killed him, or ignored the quest all together, he’ll ask for 30 rounds of rifle ammo instead. If you talk to the Bachelor behind bars, he’ll tell you that Eva is holding onto the rounds, and you can speak with her to get all 30. If you wish not to do that, simply murder two more children and you’ll free the Bachelor with only 6% reputation loss. Talk to the the Bachelor, and then return to Oyun and speak with him to complete your quest and return your rate of exhaustion to normal. Go take a nap.

(If you want 30 rounds for free, speak with the Bachelor when you first enter the prison, and then to the doghead, and either free him peacefully or kill the kids. As long as you got the dialogue with the Bachelor about Eva having the rounds, you can return to her at any point in the day, even after completing the quest, and still get 30 rounds for free. Score!)

Side Quest: “The Signal Fires”
At 18:00, you’ll receive a letter from Capella, requesting your presence. She’ll ask you to check on Sticky’s house. Once there, speak with either of the dogheads to figure out where he went. Say anything to intimidating, and you’ll be locked in and forced to fight them. A place near the graveyard will be marked on your map, go check a look. There, you’ll find Stump, and the four fires will be marked on your map. You can issue Stump simple commands; stay, follow, and light the fire. You’ll need to escort him to each of the four fires and relight them, and yes, he can be killed, failing the quest immediately. Each of the fires has guards, and going from north to south, you’ll confront the following; a doghead and two worms, two bandits and a doghead, a young man (with a shmowder!), a looter and a doghead, and finally 3 dogheads. They’ll have random loot including tyrwine, golden earrings, silver rings, bronze bracelet, feromycinum, and coffee. Enemies can only agro to one target at a time, so get them after you, and once you’ve cleared them, send Stump in to light the fires while you search the corpses. Return to Sticky’s house to find the door locked, and a young girl outside who gives you a warning. Talk to Capella to complete the quest.

(At no point in this questline are you required to go to Sticky’s house. Instead, you can go from Capella’s to Stump and back again after lighting the fires, just to save you a hike.)
Day 10
in which it becomes apparent that the Haruspex’s victory will be defined by him making the right choice.

It all ends in less than 3 days.

Main Quest: “The Udurgh”
Speak with the Bachelor after 7:00 to begin this quest. Hike back over to the Abattoir to speak with Oyun and continue your trials. This time, instead of rapid exhaustion, you’ll suffer a rapid hunger, rising to 100% in an in-game hour. Take the railway exit from the Abattoir and go to the place marked on your map. Despite his warning, you can eat, but it’s rather ineffectual. Interact with the altar, and watch the scene. The three worm huts have been marked on your map. Each place has a butcher, who will attack you regardless of what you say, and must be killed. Upon dispatching the third and final one, your hunger rate will return to normal, and you can return to Oyun. After talking to him, speak with the Bachelor, and he in turn will ask you to speak with Capella and Katerina. Speaking with Capella will direct you to Taya in equal turn, but you do not have to talk to her for the quest, only Capella and Katerina. Return to the Bachelor to complete the quest.

Side Quest: “Who is Going to Feed the Dolly?”
To begin this quest, speak with Capella at any point in the day, even before 7:00. She’ll ask you to check on Murky in her van. Before you do that, it might be a good idea to empty your “Other” tab in your inventory to your prefered place of storage. Go speak with her, and she will mark a trio of stones to the south-east on your map. Be careful on your way over, the soldiers don’t take too kindly to you getting too close to their wagon. From the trio of stones, you notice a number of sprigs on various twyr growing. Collect them and follow them down the hill and into the delta. You’ll find somewhere in the ballpark of 15 sprigs of twyr, leading to Murky’s Doll. All of this will take up space in your “Other” tab, around 6 slots. Return the doll to Murky in exchange for a blood vial, and she’ll mention that she found it at the base of the Polyhedron. Go there, and interact with the dark spot at the base of the stairs. Speak to Murky again to complete the quest. With these two vials of blood, you can craft panacea as you did on day 7.

Side Quest: “Bos Turokh is Coming”
Speak with the man in Taya’s room, and then with Taya to begin this quest. Head to the Bone Stake Lot, marked on your map. Speak the the lead worm, and then the other three. You’ll be asked to speak with four of your bound; Notkin, Sticky, Murky and Grace. Sticky will be right where he’s supposed to be, for once. Murky will be in her van, and Notkin in his warehouse. Going to Grace’s lodge, you’ll find a girl who will suggest you speak with Capella. Capella will ask you to speak with the Commander, who in turn will direct you to a wagon near the cannon, south of town. Speak with the soldier, “four hundred and thirteenth”, and then ask Grace to let them pass, and ask her about the Bone Stake lot. With that completed, speak with the three worms to receive a vial of blood from each, and then with the head to complete the quest. With these three vials of blood, you can craft panacea as you did on day 7.

(The quests today are an absolute mess, requiring you to go all over town, often visiting the same place multiple times. There’s a reason Pathologistics referred to this day as the Iditarod. Here’s my recommendation on the fastest way to visit everyone in order to complete your daily quest and get all five vials of abattoir blood and get to bed at a reasonable hour.

After 7:00, talk to the Bachelor at Eva’s home, then Capella, then Taya in the Long Block, then speak with the worms at the Bone Stake lot, then Sticky, then enter the Abattoir and speak with Oyun, use the south exit, sleep at Ragi Burrow, kill the three Butchers, go see Murky, then go speak to Grace in the van, ignoring the soldier, go grab the doll, speak with Murky and get one vial of blood, and don’t forget to ask her about the bull. After that, return to the Bone Stake lot and speak with the three worms to get three more vials, then speak with Oyun again, go to the Bachelor, talk to Capella and Katerina, speak to the Bachelor, and get the splotch of blood at the base of the Polyhedron.

Simple, easy, and you can do it in 20 minutes or your steppebux back free. This skips completing all the side quests and sequence breaks quest flagging considerably. If you somehow find a faster way, message me.)
Day 11
in which the Haruspex finds himself trapped, for his dilemma consists of mutually exclusive truths.

It all ends tomorrow.

Main Quest: “The Ultimate Trial of the Warden”
When 7:00 comes around, you may speak with Oyun to begin the final trail. However, at 7:15, you will receive a letter from the Changeling, offering you help. She is at Grace’s lodge in the cemetery. Speak with her, and choose whether or not to accept her help. Go to the Abattoir and speak to Oyun. Turn around, and in the previous chamber, the rocks will have been cleared away. Go down into the hole. If you accepted the Changeling’s assistance, you’ll see her, then be asked to pick between one of three individuals. Pick one, and talk to her to find yourself back outside the Abattoir. Return to Oyun, Foreman of the Abattoir, and speak with him. Regardless of what you say, you’ll be placed in combat with him. Your inventory is completely removed for this fight, so no weapons, no healing, and no defensive clothing. He’ll take 25% of your health with each hit, so go back and forth and whittle him down with body blows. About 30 will take him down.

If you did not accept the Changeling’s help, when you go down the pit, you shall die. However, there is a way around this. You can speak with Aspity, either Vlad Sr or Jr, or the Bachelor after being told to go down the pit, and then you can return and challenge the Foreman without dealing with the pit in the slightest.

Either way, once you emerge victorious, the quest is finished. You can speak with Vlad Sr or Jr and Aspity after the quest for some additional dialogue.

Side Quest: “Bos Turokh Departs”
Speak with Notkin in his warehouse to begin this quest. On his advice, check out the Bone Stake lot where the bull was yesterday. Speak to the head Corpsman, and he’ll redirect you to Captain Longin’s HQ, the wagon Grace was in yesterday. Longin will finger out the Bachelor. Go speak with him on the issue to finish the quest.

Side Quest: “The Spirit Will Bow Before the Power”
To begin this quest, speak with Capella and agree to help her. Go to the Bachelor, and agree with him. He’ll tell you that the password is “Fortified”. Head up to the Polyhedron’s summit and relay this passcode. Speak with either of the three dogheads, and either tell them the password, or tell them you are here on the Bachelor’s orders, and they’ll let you in to see your final adherent, Kaspar “Khan” Kain. Tell him of Capella’s offer, and then return to Capella with his verdict. You’ll receive Capella’s Necklace and complete the quest.

(Be warned, saying the wrong thing to either the Bachelor, the doghead guards, or Khan will insta-fail the quest for you!)
Day 12
in which it finally becomes apparent what it was all about.

Today is the final day. You have less than fourteen hours to make the right choice.

Main Quest: “The Final Proceedings”
There is only one task for the day. A letter from the Inquisitor will explain that at 19:00, the Cathedral will be opened for you to make a decision, provided that your Bound are free of disease, and if you’ve completed your daily main quests, they will be. You’ll receive a letter from both the Bachelor and the Changeling at 8:00, both asking the same thing; for you to speak with them and help cure their Bound. The Bachelor is in Eva’s home, but the Changeling has nested herself in the Short Block of the Termitary. Speak with them, and if you have enough shmowders or panaceas (typically around 5-7 total, the number fluxuates) you can visit their adherents, and cure the sick ones by talking to the executor outside. Be sure to visit your own adherents as well; each of them will have something important to talk about. Once you cure a set of adherents, you can return to their benefactor, either the Bachelor or the Changeling, and invite them to the council. If you cure both your own and another set, you’ll receive a letter beckoning you to the Polyhedron. Head to the top, and step on the hexagonal patterns on the floor to descend deeper and deeper into it, then have a conversation. If you manage to cure every sick Bound of the sand plague, after that conversation, you’ll receive another letter inviting you to the theater.

At 19:00, head to the Cathedral. Speak with everyone present, saving the Commander for last. Speak with him, and make a decision. You can speak with everyone again after that, and then exit. At 24:00 on the twelfth day, the game is over.

Lemons i can skip the sidequests Day 10? How you say is very annoying walk all map.

Thanks so much! You will get full credit, once I get around to adding it! A fine contribution :) :executor:

Sorry for the slow response, but if you want to add it, or request my help adding it, sure! Just credit LemonsInMyHead for whatever. I will say that the current two days written are more plot summary, while my guide is more focused on gameplay and glosses over the narrative. It might not be the best fit for what the wiki wants, but I'd be willing to help reformat my guide if it is.

This is excellent, thank you. Would it be possible to get you to add (or give me permission to do so) this to the Pathologic Wiki, which I'm sure you can see is in dire need of just such a contribution?
