I'll be updating this guide to be more accurate!
Main Game Achievements
Milestones

There are five achievements related to progression in the main game. They are:
  • Welcome! - Start a new game.
  • Surprise! - Reach Room 250.
  • Half-way there! - Reach Room 500.
  • The Final Stretch - Reach Room 750.
  • You Did It! - Reach Room 1000.

Ending Achievements
  • Welcome to the Army - Get the "good" ending by having a "violence score" of 19 or lower by the time you've reached the final room.
  • It was never a mask - Get the "bad" ending by having a "violence score" of 20 or higher by the time you've reached the final room.
Brief ending guide: Violence score consists of how many minor enemies you have eliminated with your axe; Killing a deer adds 4 points, and destroying Specimen 1/Cutouts adds 1 point..

Other Achievements
  • Marathon - Walk a lot. (It took me ~650 rooms to get this achievement)
  • Near-Death Experience - Get your health down close to death, but don't actually die.
  • New AXEuisition - Pick up the axe in the deer room.
  • Meta-gamer - Play the 3 minigames in the arcade room.
  • Avid Reader - Read all the story-related notes you see in randomly generated rooms during the main campaign. The last page will have DAY 7 written on the top of it..
  • Speedrun Strats - Beat the game without saving.
  • Specimen Researcher - Encounter every specimen in the base game. Must be done in Endless, as this achievement includes specimen exclusive to that game mode.

Secret Achievements
  • Brain Power - You need the Axe for this. If you come across the room with the brain in the tube, smash the tube and let it kill you.
  • Pacifism - Don't use your axe on any specimen other than the final boss. This will also grant you the Welcome to the Army achievement if you don't have it already.
  • Seeing Double - You get this when you're chased by two of the same specimen.
  • Game Over and Over - Die to every specimen in the main story campaign (Specimen 2-13).
  • A_Rival Mode - Beat the game with Jumpscares and Specimen turned off.
  • This isn't the End?! - Reach Room 1001 in Endless Mode.

NOTES:
  • A_Rival Mode and Speedrun Strats cannot be earned on the same run, this being because Speedrun Strats cannot be earned without the specimen option turned on.
  • A much easier way of getting Near-Death Experience is by triggering Unknown Specimen 1's second chase. This can be done by typing "OPEN" on the name input screen that occurs after dying during his first chase.
  • Notes will not spawn in randomly generated rooms if you are being chased by a specimen, so do not worry about risking your life to search for notes for the Avid Reader achievement.
  • With this in mind, it may be easier to attempt getting this achievement alongside A_Rival Mode due to the lack of specimen to reduce chances for notes to spawn.
Karamari Hospital Achievements
  • Visiting Hour - Start a new game of Karamari Hospital.
  • Knife To Meet You - Beat KH for the first time.
  • You Should Go Now - If you reach the room with the exit key WITHOUT killing the specimens with the sword, you get this achievement (earned by default on your first game).
  • She Fears You - If you reach the room with the exit key after killing all of the specimens (except the maze one) with the sword, you get this achievement.
  • Backseat Gamer - In the basement, you must initiate a chase with Specimen 4,
    and then get killed by them whilst playing Sunshine Academy.

  • Love Failed - Play Sunshine Academy and get the "Love Failed" ending.
The Dollhouse Achievements
  • Welcome to the Dollhouse - Very easy; enter the first Dollhouse room shortly after starting a new Dollhouse run.
  • Tea Time - Set the doll down at the tea table located in the West Wing and then sit down in the chair across from it.
  • Doug Dug - Get a game over ("RIP In Peace") while playing the "Doug Dug" game located in GL Labs.
  • You Tried - After the flashback sequence ends, plant the gun on the sleeping person found on the bench. (You must do this to advance to the final part of the game.)
  • Now We Can Be Together - Get the good ending. (Escape the mansion after sealing the Hell Well.)
  • Demons Walk With Us - Get the bad ending. (Escape the mansion without sealing the Hell Well.)
  • You're In The Know, Right? - It's complicated. These videos (Video 1) (Video 2) demonstrates how to get the achievement, but the general steps are:
    1. In The Great Hall, go to the right hall.
    2. Turn left and walk into the wall. This will lead to a hidden room.
    3. Another secret passage lies within this hidden room. To reach it, take the left path and keep going until the path reaches a U-turn. Upon reaching the wall, turn left and walk through the wall again.
    4. Walk up to the display containing the text and interact with it.
    5. Enter "INTO THE SEA YOU AND ME ALL THESE YEARS AND NO ONE HEARD" and then Confirm.
    6. When the Game Over screen appears after the passage of text disappears, you will get the achievement.

  • You Fool - Trigger all self-inflicted deaths. (This video) demonstrates all of them, but the general list is:
    - (Missable) Die by attempting to open the barbed wire door in the Northside Studies.
    - Die by sticking your hand through the hole in the wall in the West Wing.
    - Die by running into the electric fence while it is activated in The Great Hall.
    - Die by repeatedly attempting to open the Furnace while it's running in The Great Hall.
    - Die by failing to reach the Tram during the self-destruct sequence in GL Labs.

Judging from the achievement's progress bar, no. Only the main 12 specimen (not including 1 since those cannot kill you) need to kill you.

you dont need to go to the tram before the facility activates it's self-destruct sequence and die by specimen 9 (if you're doing the "you fool" achievement ofc, i forgot to say this before). once again thx for the achievement guide, it helped :D
