You haven't done Dead Center, The Passing, Dark Carnival, Swamp Fever, Hard Rain, The Parish, The Sacrifice, Cold Stream, or The Last Stand campaigns in Left 4 Dead 2 (basically, you haven't completed any campaigns from start to finish, unless you completed a L4D1 campaign, since those don't have achievements). You haven't completed co-op in Portal 2.
