"Slow Decode" - Install the DirectX 9 runtime for DXVA support!
If you're trying to stream to a low-end Windows machine from the last few years, make sure you have the DirectX 9 runtime installed. "Slow decode" errors mean your client's decode stream probably isn't hardware accelerated. That, or your client PC is too old.

Grab the DirectX 9.0c runtime from http://www.microsoft.com/en-us/download/details.aspx?id=35. You can also grab it from your gaming PC, out of a redist folder of one of the many, many, many games that tries to install it on first launch.

If you want to see if hardware accelerated DXVA decoding is active, open up streaming_client.exe.log in your client PC's Steam folder
(usually "C:\Program Files (x86)\Steam") and look for "DXVA". If it says "init failed", it isn't hardware accelerated. If it says it's using your GPU, it's working.

I have DXVA working on an Intel 4 series chipset but eventually I get "DXVA: Out Of Memory Buffers" and "libav error: decode_slice_header error" appearing in the logfile.

Using latest Intel drivers, etc.
