Ok, my final answer. There will be only three girls who will stay with Touma.

Index will win, since the title is called Index. Also, she's the one who kissed him first.

The series is not about Romance it's about Science and Magic. If the author will put some romance in the series, that would be hard. Many fans wanted Mikoto and Touma to end up together. But what about Index and Othinus? Will Touma leave them behind? No. Index had feelings for Touma. I'm not sure about Othinus but their relationship between the two of them is beyond love and friendship. Othinus really liked Touma. Also, Index is a nun, will she abandon for being a nun? If that happens, then she will surely end up with Touma. (Well the series itself is called Index) But what about Mikoto's Jealousy afterward? She will get over and remain friends/allies with Touma and keep supporting him till the end. But seriously, Touma will be having a hard time choosing the three of them.

Will Mikoto ends up with Etzali?

No. She doesn't have interest in him and she barely knows him.

Will Kuroko ends up with Tsuchimikado?

Not a chance, she sold her body to Mikoto and the only guy she likes is Touma.
