Do idle games count? I have a couple that require years of play (not in terms of hours, but total time since starting the game).

Just based on games tracked within Steam (there are a couple idle games that I played before they moved from Kongregate to Steam, so I don't know exactly how long I played them before)... my longest achievement is in Idling to Rule the Gods, taking around 4 years (and about 25000 hours of leaving the game open). And I haven't even 100%'d the game (the in-game progress tracker says I'm in the "mid game" :D ).

Ticket to Ride, the "Conductor Level 5" achievement. It took around 1,500 hours to play 20,000 games. I had to create a bot to play the game while I was asleep or at work. None of those games were wins, of course - the thing was only programmed to draw cards. But the challenge was to play games, not win them. It took around three to four months before the cheevo popped. I wrote a guide to help others with it, too.

Maybe Blood Money from counter strike source. Earn 50,000,000 in cash. I usually don't bother with idle games. There's also Payday 2 getting to infamy 100. That took longer now that I think about it. I never did complete the last trophy in that game. I forget how many kills you need.

Any conditional achievement involves with Kill X amount, Earn Y amount , etc.

The "No gold" runs in Spelunky and Spelunky 2 took me absolutely ages. Still great games but man I had to take a break after those.
