What makes this one difficult? I haven’t played it myself :)

The hardest game I've ever 100% completed is Action Henk which is game focused on going fast and time trials. I had to fight tooth and nail for every half a second for those rainbow medals, but it was worth it. It was so hard that I was in the top 200 players on the leaderboards when I finally completed it.

Other notable time-consuming and difficult games:
PowerWash Simulator [78 Hours] - With every DLC, that number gets higher. The game is good, but man, does it start to overstay its welcome with each new paid DLC adding more achievements. The DLCs combined cost more than the game itself at this point.
Last Command [17.9 Hours] - Beating the final boss on the highest difficulty made me rage quit twice on the same day I beat it. Snake and Bullet Hell is a surprisingly genius combination for gameplay. (I'm infinitely grateful that you can just play the game on normal mode and fight every individual boss on the highest difficulty afterward without needing to replay it).
Shovel Knight: Treasure Trove [77.1 Hours] - It's essentially four main games and a spin-off collected in one package. The achievements for Showdown were annoying (even if they were surprisingly possible in singleplayer), but the rest was great.
Jet Lancer [26.2 Hours] - The combination of a very strict time limit forcing you to be reckless, needing to beat each level without getting hit, and the sheer amount of enemies from every direction the game can throw at you makes this game ridiculous at times when going for the 100% completion. It's fun, but maybe don't force yourself to complete it.
Umineko When They Cry: Question Arcs [71.6 Hours] - I don't count visual novels as completed unless I actually read them, so those 71 hours are a testament to how long this story is. Mind you, this is the first half of the full narrative with the other half split into a second game (which I haven't read yet). If they were combined, this would be the definitive most time consuming completion. It's really good, but I'd recommend something smaller like Slay the Princess as an introduction to the genre of visual novels over this.
(I've completed 260 games so far with more examples that fit the criteria, so I'm going to stop now to make sure this post actually ends).

I personally haven't heard of cross code or action hank but ill check em out here after I write this.
Powerwash simulator is definitely interesting to me, I have never really been into those kind of games but i can totally see how people can get wrapped up in it, looking at you farming simulator.
Last command and shovel knight look good, I haven't played them but I have seen other people(youtubers/friends) play them. I wouldn't mind getting shovel knight here soon as it's totally up my alley.
Jet lancer I am actually working towards right now! The game had a really good sale recently and I have been enjoying my time with it so far, The pixel graphics and fast and intense gameplay make for a great experience IMO. Not to mention my love for aviation.. even if it is a little unrealistic.
I also haven't heard of umineko but I will give that a look here soon as well. On that note I personally count visual novels as games, just because It might be straightforward and easy or only have non-missable achievements, I don't think that should discredit it as a game. One example of this is a couple of Telltale games I have completed. The walking dead series was really good and even though it kind of is an interact-able movie, I still count it.
260 games is pretty crazy man, that's something to be proud of for sure. I appreciate the input and keep up the good work hunter!
There are generally two reasons. The first being multiplayer - especially fighting games with rankings. If I can do multiplayer matches with a buddy it isn't as much of a problem. But Street Fighter V... well, I can't finish it simply because the last achievement requires me to hit Gold rank. The highest I've been able to hit is Silver. And unlike the other Street Fighter games I've played, this one can't be boosted with the help of a buddy. I know that a lot of other players think that Gold is easy, but I don't have the time or the patience to play online for hours and hours in order to "git gud." And with Street fighter 6 out, there aren't that many players in V any more. The other achievements - including beating the survival mode on the hardest level and winning 300 ranked matches - were easy for me by comparison.
The second thing that trips me up tends to be time challenges, or speed runs. It's the reason why I haven't been able to get 100% completion in Braid yet, and why I avoid a lot of other games that have similar achievements. I would be crushed to play my best through a game only to find I miss the goal by a few seconds. It actually would discourage me from trying again.
Yes, there are some games that I wouldn't be able to finish simply because I'm not skilled enough to beat it at the hardest setting. That's fine - I know my limitations. But those two above... those are the ones that I find the most difficult, perhaps because I also find them the most frustrating.
Yeah I feel you on the multiplayer achievements for sure. There are definitely some crazy hard multiplayer achievements out there, and as you mentioned the player-base has to be alive to a certain extent or else it becomes near to impossible. I understand why many games have multiplayer achievements, especially ones that are centred around multiplayer instead of just being an additional game mode, but It doesn't take away the negatives. I have heard of some developers choosing to have a way to unlock some multiplayer achievements solo by creating a harder requisite... Personally I haven't played a game that does this but I am a big fan of the concept.
Time challenges on the other hand are super stressful but I enjoy the rush they give me ngl. I have completed a few speed runs for games like Chained Together(2hrs) or Kingdom Hearts 1(15hrs). With that in mind I have been lucky enough to not have to retry any of these thankfully so that might be why they haven't frustrated me yet! I am sure it's bound to happen to me at some point in my achievement hunting and when it does it might change my opinion.
With this in mind, my original question/this discussion was more so what is the hardest game you have completed, would you care sharing that?
I would have to say "Ticket to Ride." There were some extremely difficult achievements in that game that required you to get just the right cards at the right time, and have a lot of familiarity with some of the game boards in order to get them. They require playing the game in rather unconventional ways - which is an approach to achievements that I actually like. Sometimes developers require you to do weird things in order to get an achievement - and it might not be in a way that leads to a winning strategy. It's kind of a kick to try and do weird things in order to succeed, in my opinion.
A close second would be "Queen's Wish: The Conqueror." It's the first Spiderweb Software game that I've been able to complete on the hardest difficulty setting (and thus getting all the achievements) and I managed to get it without having to rely on in-game cheats, too. I'm particularly proud of that accomplishment, and even wrote a guide to help others try to beat the game as well.
Nice! I love Ticket to Ride, me and my family play the board game all the time. The different maps are super cool and I like how they are real locations. Honestly, I might consider getting it on Steam now that it's in my mind lol.
I haven't heard of Queen's Wish but I'll take a look for sure. Writing a guide is something I have yet to do but I use a lot of them while hunting so I am greatly appreciative of those who put in the time to help others with a guide.

Take note that I got the achievements (and wrote my guide) for the "old" Ticket to Ride that is no longer for sale. From what I've read, the new version still has several flaws with it, and they haven't released all the maps yet (which means that more achievements may still be incoming).
