When I'm trying to pick items latter in the game, it swaps two times by itself and item on me stays the same. It happens when trying to swap from epic to legendary and just items, not weapons. Not sure is it a bug or just me

so far i haven't had this happen to me, i have always been able to pick everything up and/or swap them. i've played maybe like 10 hours after zone 3 and haven't had pickup/swap issues, only having issues with ghost blade not cloaking me after kill.

Also had this bug today, I couldn't pick up anything at all. I saved the progress in upgrade room and restarted the game and then I could pick items up again.

by any chance did you minimize the game? if yes, then most likely there's your issue. don't minimize.
i did it today by accident and other time i needed to check something, both times the item locations didn't correspond to the icons over them, so the highlight was in wrong place. it seems like game itself and game's overlay (HUD) are rendered separately, so when you minimize, their scale goes out of sync. you can usually fix this by setting game between window and fullscreen til it fixes.
not sure if that's what you had, but thought it's relevant enough to add it here.
