If a player joins my game and I see them literally cheating (e.g. unlimited ammo, "god mode", etc.), they are banned from my games no questions. All it takes is a vigilant host to monitor every teammate to make sure they are not doing something suspicious.

This is basically the answer. Mind your own lobby, play the friggin game. Obsessing further is unproductive.

On the load screen you should have a
"mission save"
"pre-mission save"
"per act save"
Now dyeing deletes the "mission" and "pre-mission" save I believe leaving you with only the per act save. Unless of course you are playing real perma-death that will delete all of your save files. I'm wondering if with the boozer glitch you can hit "escape" and "quit mission" resetting you outside of the fire watches wire perimeter like the other posts imply.
