Well, before the likes of game clients and buying online, first one I had to use a game client for was Skyrim, although I spent a long time trying to find a way not to.

Before the advent of game clients like Steam, you used to either have a manual with the game or the opportunity to buy one. Which in the case of Morrowind I had to as it was driving me crazy. The manuals told you everything you needed.
I believe Diablo 2 manual was over 50 pages long.

It is a lot more difficult nowadays to put all the info into tooltips and little write ups.
Look at the tutorial on Dodge with this game, it says press spacebar to dodge.
Ok great, but what does it do. You have to test it. Maybe new players do not realise this is the test area to try it out.

I am trying to see all sides here.
Not bothered with those who know aRPGs and the way they work, they should know not to whinge.
It is the new players to this genre or games I would like to help.
In the words of Cranky Kong I've been games long "enough to remember when falling in a pit in a platformer was called 'lack of skill' and not 'cheap.'" There's a plethora of options in the game and the game explains its mechanics in much more detail than Titan Quest did. New games all included manuals, so you must be referring to strategy guides which are an entirely different beast (and more often than not published by third parties).

Frankly, if you're saying pressing a button is too hard for players and that players don't want to try out new abilities they get then they're terminally incurious and won't enjoy any sort of game that isn't a walking simulator.
I will say there's a point where "lack of skill" becomes "cheap". Using your pit example. Failing to jump over an obvious pit is lack of skill. Failing to recognize that those textures are different and it's probably a pitfall trap (likely hinted to you earlier) is also a lack of skill. Dying to a pitfall trap that had no indicators, no prior experience, and no way of knowing it was a pitfall trap prior to dying to it, is cheap. And Dark Souls is littered with "hard" content like this, which is why I say that Souls Games are "frustrating" not "difficult"
