Bans are delayed so what got detected could have been from weeks or even months before the ban was issued.
Most likely, but I had the skinchaser for such a long time (2015 is the date of its installation and the date of its removal), so I don't understand how it could have received the VAC after such a long time. I don't even think there are any logs left since then :)
Only Valve would know for sure, and yes they have detailed logs about every VAC ban. However none of us or you will ever see them.

No. We do not disclose the cheats that were detected while connected to a VAC-secured server that resulted in a VAC ban. We have detailed records for each VAC ban, however, releasing this information would only benefit cheat developers. The VAC team regularly investigates claims of false VAC bans to increase the effectiveness of Valve Anti-Cheat.
