What does +rep and -rep do
I see people comment "+rep" and "-rep" on profiles all the time, sometimes followed by a reason. Is this a thing or command that Steam actually tracks, or is it just a thing people glance at to see how trustworthy a user is?

Nothing, and it's not tracked by Steam. Not sure the actual origin of it but it's an informal thing that caught on, and some people are misinformed and think it actually does something and do it under the impression it has meaning. Some people do it as a joke on friends or strangers' profiles. A lot of scammers use it to make themselves look more legit.

But seeing as you have full control of what comments are on your profile, you should absolutely NOT take it as a reference to whether a person is trustworthy or reputable whatsoever. Seeing as you can simply delete any negative comments, "-rep" comments included.

I was just asking because someone commented it on my profile after we had a single brief conversation in off topic and I was just wondering why and what it meant

For the record, you can leave it up if you want. You won't get in trouble for having +rep comments on your profile. Just don't use it as a way to determine if someone else is trustworthy.

-rep may annoy kids that care about this stuff

+rep makes whoever you give it to stronger.

It's a psychic way of making their body think they did an extra rep as a workout therefore making them stronger without having to do anything.

It does absolutely nothing and is a huge red flag if you see them on any steam account . People cherry pick by deleting the negative and keeping the positive .

It's mainly used by scammer's and bad actors to build a fake profile and make it seem like they are trustworthy.
As there is no tracking system for it, it is worthless.
Especially because users can just delete the negative ones and only keep the positive ones.

It has largely zero effect, beyond what others noted above, it was for a long time used by TF2 and CS2 traders to provide who was trustable when it came to trading items, outside of that it has zero use because Steam in of itself does not track it, only third party sites do but until you sign into those sites it literally holds zero effect. (Found that out from a TF2 trader)

With how easy it is to delete any negatives, that whole system was useless.
That's why no one trusts it.

As already mentioned, it's a useless reputation system.

Not everyone who uses it has bad intentions. The comment on your profile is harmless.

it does nothing, its spam.
It’s just words to me. It doesn’t really mean anything but people believe if they have +rep in their profile it means they will be more favored.
