Actively scary gameplay would be Alien Isolation probably.

But Soma and Silent Hill are AMAZING and should be played by any horror fan.

Bonus mention for Voices of the Void but that game isn't on Steam yet.

I can't really decide what is the "scariest" game I've played, but I'll give a few examples of some really scary experiences:

Amnesia: The Dark Descent
Resident Evil 4
Inside
FNAF: Help Wanted
DOOM 3 (I haven't finished this one, but from what I have experienced it's pretty spooky)

Alien: Isolation or.. Outlast 2.

Either way, I had to drop them because of that.
To this day, I'm scared of getting back to them, nor do I plan on doing so.

Soma is worth mentioning, but due to the zero or nearly zero consequences at lower difficulties, it was manageable.

Agree with Alien Isolation. That was hands down the most terrifying experience I've had in a game. Highly recommend. If you can play with all the lights off, or in the VR mod, than you have ascended to the next dimension.

I said Outlast 1 and 2 , mostly 2 but i hear people mention Soma. i think Soma is a different kind of scary but still scary as a mongoose in heat.
I think of Soma alot. That is gonna happen ,putting consciousness in machines, probaly already is going on.
Reminds me of that show Black Mirror. It is frightening. Half of that stuff has already happened and the rest might happen too.
