The game is themed as rats wanting to get their revenge on cats, and thus the target in each level is a cat, who has to be maimed, killed, or otherwise humiliated. This is typically considered distasteful, of course.
The game doesn't work particularly well as a physics sim, in part because (1) the playing field for arranging items is in 2D, but the physics engine works in 3D, and (2) there are random effects that mean that simulations actually don't produce consistent results. The game is generally considered to have poor gameplay.
Due to the game's low price, however, it gained a status of infamy, because people started using it for practical jokes, such as gag gifting. (Secret of the Magic Crystals is another game that's a frequent gag gift.) In fact, Bad Rats has been purchased so much that the devs are making a sequel.
