Gear based - grindy- good economy/crafting MMO(rpg)?

Hi everybody, I'm sure you're tired of the question "what mmo should i play?" but I am really LOST. I'm not even sure now if a mmo like the one im looking for exists. I'll just put down a list of things I like and I don't like and hope you guys have some ideas for me.

Things i like:

Gear based (I REALLY like when there are exclusive items, gained through hours and hours of grind or whatever, and I don't mean vanity items, I mean stand-out items)

Good economy

Good crafting system

Difficult dungeons (end game near to impossible, which requires tactic and preparation)

Cash shop which let you convert real money into virtual money easily (you may call it pay2win but I actually like to have this option, gold sellers exists in every mmo and I prefer it to be legal!)

Grind (both exping and collecting items)

nice community

populated

massive pvp

Things I don't like:

WoW model (mainly the quest way to exp, i find it more boring than grind)

easy istances made just because people want to be able to beat every mob in the game. IT MUST BE HARD.

Just to give an example, I've played plenty of mmorpg but the one I've absolutely loved is Ragnarok Online.

I have played Guild Wars 2 for 3k hours maybe when it was released and I don't know man, I'm thinking about GW2 when reading your post. It was a mmo that wanted to innovate when it released.

You literally just described Guild Wars 2. It also went F2P recently and you can actually get max level with a free account unlike WoW.

Gear based, emphasis on crafting, dungeons are a huge part of the game and has a built in currency conversion system that goes both ways! Buy gems with cash that allows you to buy premium cosmetic items or sell your gold earned ingame for gems or sell your gems for gold for price you want on player market.

You can also buy the core game for about 10 bucks online or just wait for the expac to go on sale during the holidays.

Final Fantasy: A Realm Reborn has pretty much everything you wanted. The crafting classes are super grindy...think that owes itself to the fact that you also have to level a gathering class or two as well.

Dungeons are rather difficult, the raids can get insane with entire party wipes for one little screw up.
