What features make a monster actually scary in a horror game
I’m making a horror game and I’m in the modeling stage for my monster. What are things it can do and features it can have that will genuinely be terrifying

A monster who can 1 shot you. But here is the kicker… it will be blind but will be super fast and super hard to kill.
well there are the traditional genetic fears I'm not going to list since if you put them all in a game at once you could kill someone.

Ah yes, and don't forget the obligatory game design decisions where you will have to make noise in some way to advance like activating a device or moving an object which will attract said monster xD

Put various blinking eyes on its face.
In a game called “Dead Frontier 2” there is a zombie that will spawn randomly. You don’t know when because it’s RNG. This specific zombie is unkillable and will chase you around until you go back into an outpost.

They should make it scarier by allowing it to instantly kill you. And yes… it does run.
Convulsing mutilated pile of flesh.
Like the Glutton in Sillent Hill 3.
A monster that sneaks up on you deep in the ocean. A la Subnautica.
Have the monster repeatedly ask to speak to your manager
You could have an ever present threat. Make them feel that no place is safe. I think Resident Evil 7 did a really good job with this. Having to stay alert even when doing puzzles.

The scariest thing is a decent AI... like in 'Condemned' or 'Alien Isolation'.
Everything else is optional.
With a good AI the monster could be even a cute kitten.

Oh yeah that's a good point. A monster design could be really scary but that doesn't mean much if they are janky.
The monster has to be somewhat unpredictable, which is difficult to do in a game.
I'd say you have to make the monster change over time in either appearance, grab animation, story wise, game machine wise, or all the above.
Monsters tend to lose their appeal after the first few times it attacks you.
