Older CoD games were hard mostly for the grenade spamming, Perfect Dark on the 360 was pretty hard too.
Did you actually use Vita Chambers in BioShock? Cuz it kinda doesn't matter if you die when they are enabled. As for Lovely Planet, definitely need to check it out.
That's correct, no healthpacks and no checkpoints at some points if health is lower than 33%.
I didn't play on 1999 mode or whatever, but I didn't find Bioshock that hard, I literally never used the Vita Chambers even once. This was on the 360 version anyway.

Were you playing on Hard?
Also, 1999 mode is Infinite, and I played it for about an hour before saying "this is completely imbalanced and totally ♥♥♥♥♥♥♥♥" and stopping playing. Before you all call me a ♥♥♥♥♥, it was seriously kinda ♥♥♥♥♥♥♥♥. Literally all of my ammo to kill a fireman, who can kill me in two hits? No, thank you.
No 1999 Mode in the first 2 games, but good job on getting through without 'em :)
Hmm don't remember Infinite being that tough, there was always a trick for everything, it was usually being in cover all the time and picking guys 1 by 1 with headshots. The last bit however was tough, that's where you truly needed your vigor powers.
I thought there was a 1999 mode or something similar in the first BioShock? I must be mistaken. It's been years since I played it, I think it must have been on Hard mode, yeah. I remember getting the Xbox achievement for not dying.
I never touched 1999 mode for Infinite, just Hard. The only time I died was fighting the ghost of Comstock's wife. At least twice. I understand most people found the game (in general) easier than the first two BioShocks. Yeah I agree with you, that 1999 mode does sound like BS balance, ♥♥♥♥ bullet sponges and cheap enemy kills.
Never played BioShock 2 for more than an hour or two, hated it.
Anyway OP, I don't play many games in singleplayer mode anymore but from my experiences I would say
1: CoD: WaW
2: CoD4 (because of that plane mission at the end... that mission left a scar on my sanity because of how many times I died)
3. L4D2 (I've only survived one campaign on expert so far)
4.BioShock 2
5. Borderlands 2 UVHM OP8 + the 4 player slider I use to get better loot drops. It's not really hard, but the enemies soak bullets for days at this point.
