"landmine goes click" (2015) in terms of tension, "grotesque" (2009) in terms of shock value. "a serbian film" (2010) is also shocking and exploitative, but not very good.

"ichi the killer" (2001) is great as well, and to me a positive example of extreme cinema. "funny games" (1997) is highly disturbing and comedically dark, and is sure to be unnerving viewers.

i have also seen some of the guinea pig series, most famous of which is "flower of flesh and blood" (1985). it is definitely violent and shocking, but it has very little substance.

I don't think I've seen any really crazy movies in recent times, but I remember Zathura: A Space Adventure was pretty trippy at the time I saw it. That and Spy Kids 3 or Sharkboy and Lavagirl.

Early 2000s was fun for my child brain I guess.
