If you are just looking for unarmed combat, there are always the Street Fighter or Mortal Kombat games. But those games aren't really realistic.

Chivalry is really interesting and To Be Honest reminds me of "Thief (DarkProject)", for that game's initial concept was like that of Chivalry, only back in 99 or so, but this more or less "aside" for it was the Melee -iDea that sparked my imagination (a [long] while back).
Realism in these kinds of games (even FPS's) all comes down to:
1 Environment (map design, "Engine" content & Flexibility)
2 Ragdoll, especially this if it's "Realistically Simulated" as in Real Life, then we've got ourselves a really potentially interesting game (as in Sim.).
However, now that we have games like Dark Souls where the fighting is more grounded and realistic in comparison to older games. I think a first person melee game made like Dark Souls can work surprisingly well. Devs have really been nailing the physicality of a first person system lately and I think the game you are looking for might be right around the corner.
It is actually a pretty good idea.

Still would LOve to see such a game to exist. Ninja FPS/Melee Simulator... With "dirty fighting" and stuff... Oh man. Just the controls would be worthy of a "simulation" title. I bet it'd be _good_.
You would have to take the roll out though, or you would have people puking. Replace it with a dash and you are golden. Then the biggest problem would be jacking graphics detail up and making animations fit the new perspective. Biggest problem after that would be how to detect enemies that are outside of eyesight.
I am very unfamiliar with Dark Souls To Be Hones (nota console "fan" but they're ok, as in HUge market,).
My point with Dark Souls is that it did a good job of slowing down melee combat in games and made the core principles of fighting important again...attack, counter, parry, block....this is the kind of stuf fthat would work in first person.
The thing I was suprised about the most with Dark Souls was that it almost became a 3D fighting game in PVP.
...as this concept has had the time to "ripe", right? (judging by the History).
Chivalry is really the game that comes to mind. Other than that, melee focused first person games are few and far between. Look how much people love competitive first person FPS titles...if someone makes a title like that for melee combat with better combat, they will be swimming in a pool of money.
PS. Dark Souls, Wow!! (but "alas" it's a Franchise meaning... well worked out, you know, and stuff)
