The AI in games like Phantom Pain is just atrocious. I swear the guards have glaucoma and brain damage. Honestly it's just Just Cause 2 but with a really generous stealth system stapled onto it.
Hitman 2016 was a clunky console port, and the disguise system is semi-broken. Even a non-stealth game like The Saboteur or New Vegas does that mechanic far better if that's what you're looking for.
Dishonored looks cool, but that's less of a stealth/ immersive sim than it is just an action game with stealth elements. It's a far cry from Thief, which it was clearly trying to be. Thief 1, 2, 3 are probably better options if you want immersive sims.
Deus Ex and Splinter Cell are probably the best pure stealth series I know of. The stealth mechanics are really refined. Styx has its moments too.
Oldie but goldie. The Predator and Alien story campaigns are all stealth and ambush.

Another stealth and ambush game.
Metro series can be a KO fest. The way the work out the good ending Last Light is a bit obtuse. Though in Exodus you can scrape out a good ending if you only murder one of three factions people. I spent a lot of time KO'ing mutants and Peter Pan rejects in that one.
The enemies will ignore you until you are ten feet (or about three meters) from their face. I've seen enough footage to know this isn't a fluke. Also you can ride your horse within view of an outpost and they completely ignore you. And the invisibility-cloaking cardboard box is ridiculous. I'm sure it's amusing, but as a hardcore stealth fan I have to roll my eyes when people point to that as a selling point. It's baby-proofed stealth on the level of The Last of Us.
I compare MGS V to Just Cause 2 because the enemies in JC will hunt you to the ends of the earth and can often spot you before you see them. You have to literally get out of sight to lower your wanted level. You play too sloppy and snipers, anti-aircraft guns, rocket troops, and paratroopers will kick your butt. The risk is so much higher and accomplishment so much more rewarding. JC isn't even built with stealth in mind, yet it's probably a better stealth game.

Honorable mention: Metal Gear Solid V: The Phantom Pain.
Splinter Cell and Hitman are important to the genre, too, but I don't like them.
