Angle, most probably, with lateral tension during use coming more often from your left.
Unless you'r left-handed or not particularly dexterous with your right-hand fingers.
If you're holding the dispenser in your left-hand, which is likely if you're right-handed, then the extra size of the anchor point helps you secure that end a bit better, too.

I only ever lose tension on the outer hand myself...

On a side note, I use floss efficiently so I only have enough length to hold the ends at my finger tips to save resources.
However my wife who is a dentist would use it sparingly and wraps loops around fingers for better control and firm tightness. She's like a dental "hitman" with her floss.
so I'm always sure to grab like 20% more than I think I'll need to avoid using 200% more.
Also, you should be wrapping your middle fingers with the floss, so that you can use your index fingers to control it better.

I can't stand using those things. And as a long-time restaurant worker, I also can't stand those things being the main source of litter around the exit doors of a restaurant. Put them in the trash, or your pocket, slobby mcslobbersons.
