The original ending in postal 1 the final level was shooting up a school of children, it was removed because I guess even the devs thought it was too much and didnt want the game to get an adult rating or be banned so in the end instead you go in, try to shoot the children but they are seemingly ghosts and then you find your own grave near a church realizing you are dead.

(probably misremembered it to some degree but that was basically it)

I'm still surprised rimworld added kids, it was against adding them for the longest time.

That game itself can be pretty shocking.
