Hmmm. That is one heck of a question!
I have to think about what the mix really is of those factors- and of course, not every individual is the same or has the exact some circumstances.
Perhaps, the most (subjectively) accurate answer is deep down someone potentially successful knows that they have something that others don't and can pull ahead- to some point.
A great tragedy is that probably most who have gifts never fully bring them to bear. Unless someone was born with full blown mania it usually takes something external to light that fire- and most people when hit with negative stimuli crumble- so scratch the majority off the list.
There is a theory that we would have colonized space by now if every human hit their full potential- but clearly that is not the case.
Even in my life, I grew up too upper middle class and comfortable and didn't find my fire till 15 and then really went full on killer-diller in my 20's.
I will make a series of YT talks on these subjects and one example is the difference between the armies of waiters and the person making 20 million a picture is the one who made it got laughed out of 100 auditions. The waiters quit after 10.
I have frequently cited that the 6 major sharks(and virtually all of the guest sharks) experienced poverty and the rich kids who audition almost all will blow millions in funding and it is the poor applicants who are the future captains of industry as they don't have the luxury of screwing around!
So yes, there are superstars who grew up comfortable- but they aren't the norm. Usually it takes something to light that fire.

It's a question of stimulation and getting things heated up.
People commonly think that if one does a good job people might like them and if they do a bad job things also will equally bottom out:

And it isn't. That is not how it works.
It is more like when one pushes enough buttons and is getting somewhere one will experience more love andhate.

Think of every famous athlete, celebrity, politician... they not only get gobs of praise- but at the same time face withering criticism, ridicule, trash talk...
What has happened is they are there because they got people hot!
sometimes external pressure works the opposite way, where someone who initially wants to give something to the world and "help people" as a whole ends up deciding the people they want to help dont deserve it.
more fundamentally, someone who has enough internal pressure to motivate themselves is only going to be slowed down and annoyed by external pressure. of course this comes at no loss to those administering the negative pressure because they by nature do not appreciate what they do not already know to exist in the first place.
Those are some interesting points- and again, not everybody is the same or responds to events equally.
Mania is the central factor- and the word needs an image rehab.
Anybody who is creative, a bodybuilder or athlete, a performer, a business leader, politician, good in bed... has some degree of mania. That is what causes them to stand out and not just be another drab nobody.
To answer the last question, the majority respond to the negative- but it is possible that if the fire is strong enough, some are going to get it going on their own or more from the love side.
That is my most accurate observation.

There will be used in an upcoming YT talk about subjects such as this. The focus of one of them is that once one hits enough of a reaction they will get more intense feelings of all sorts- and to expect it.
Making things such as them is actually fairly easy with many common image programs.
