First, those who have the skills and ability to actually make a good game are less likely to actually make a game of this kind. If you are a talented game developer, you are very likely a professional in the field, and as a professional you may not want a pornographic game listed as a potentially black mark on your portfolio.
This leaves amateurs, who aren't as concerned about their professional reputation as a game developer. Trouble is, amateurs are unlikely to have the skills to make a good game, pornographic or otherwise.
Then you have the con artists. Basically swindlers who churn out a huge amount of crappy asset flips with hasty AI generated artwork. They're not interested in creating art or making something worth playing. They're just hoping the presence of tits will part enough fools with their money to make their minimal effort worth it.
Unfortunately, the most promising examples of adult games seem to be designed not to actually be completed. They're just a set-piece propped up in an effort to get people signed up to the Patreon. Once a monthly income is coming in via Patreon, there's actually incentive never to finish the game, since a finished game means there's not much reason to stay subscribed to the Patreon. They'll update it just enough to keep people on the hook, and keep them paying out monthly in the hopes that they'll eventually finish it. They never will.
I think the best adult games are probably coming out of Japan, but even then there are issues. There are some decent amateur efforts, but they're created with minimal or no actual budget. There are some higher-budget projects, like what Illusion (now known as Ill Games) was putting out, but these seem never to get past basically what amounts to a prototype stage, dressed up and sold as a product. Professional developers working on them are only using it as a springboard into "real" game development, and they're in a hurry to leave it behind.
To me, the best 18+ games aren't even 18+ games. They're just good games that have been modded into 18+ games, like can be done with The Sims, Skyrim, and Fallout. You have a foundation of a decent game that's actually complete and fun to play, then bolt on the ass and titties afterward.

Fundamentally, it's a competitor to The Sims, led by Rod Humble[sims.fandom.com], the guy who was the lead on The Sims 2 and 3 and who worked on Second Life, with it's own selection of adult content. What makes it interesting is that it's not an 18+ game, but it's built from the ground up with modding in mind, with built in modding tools, and uncensored nudity is an option by default.
Rod has basically confirmed without actually outright saying that Life by You is being specifically made to facilitate 18+ mods. The game is entirely offline, with no telemetry at all. He's mentioned that people often use these kinds of games to experiment on their own with sexuality, and he wants to ensure people feel safe doing so and have privacy.
If Life by You takes off, I suspect it will become the ultimate 18+ gaming experience, with all manner of specific kinks and interests catered to by the community.
That goes for AAA games as well, except they look better.
