"unfair" achievements and "difficult" achievements are two VERY different things.

-for example techincally the most unfair achievent i got is one for finding a secret tv in a team fortress 2 map that pretty much no one plays in, making it almost impossible to get.

-in terms of mechanical skill, the most difficult one would be completing chapter 9 of celeste, its post launch free dlc that was made to be harder than the base game.

I'm not a extremely dedicated hunter but the hardest I've unlocked is "Skilled" for S Rank MGS Ground Zeroes, "Solid Gold Baby" in GTAV, and maybe "Adaptable Victory" Into the Breach. They're around 5% achievements I believe.

I’d have to go with beating Getting Over It for the first time.

It took me 44 hours total to do. And that comes down the fact that certain parts of the mountain are magnitudes higher in difficulty than everything else.

It’s the type of game where some sections can be cleared consistently, while others can take hours just to pass even a single time. From the moment I reached Orange Hell for the first time, to the moment I finally managed to get past it and reach the top of the church, 16 hours had been added onto my playtime tracker. That’s nearly half of my run time on that singular obstacle.

It’s one of the best examples of “NES Hard” game design being applied to a modern day game, in my book.

No death runs on the highest difficulty settings, usually.

Recently started adding in perma-death into games as well.
